摘要
目的 探讨养心健脾汤治疗不稳定型心绞痛(unstable angina, UA)的疗效及对血脂、血液流变学指标的影响。方法 选取我院UA患者80例(2023年1月至7月),随机数字表法分组,各40例。对照组给予西医常规治疗(阿司匹林+阿托伐他汀+单硝酸异山梨酯片),在此基础上实验组给予养心健脾汤,两组均治疗14 d。比较两组总有效率、硝酸甘油停减率、中医证候积分、血脂[总胆固醇(total cholesterol, TC)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol, LDL-C)、三酰甘油(triacyl glycerol, TG)、高密度脂蛋白胆固醇(high-density lipoprotein cholesterol, HDL-C)]、血液流变学指标[纤维蛋白原(fibrinogen, FIB)、红细胞比容(hematocrit, HCT)、全血黏度(crude protein, CP)]。结果 实验组总有效率90.00%高于对照组72.50%,差异有统计学意义(P<0.05);实验组硝酸甘油停减率87.50%高于对照组67.50%,差异有统计学意义(P<0.05);与治疗前相比,两组治疗14 d后胸闷、胸痛、头晕、倦怠乏力均下降,且实验组低于对照组,差异有统计学意义(P<0.05);与对照组相比,实验组治疗14 d后TG、TC、LDL-C下降,HDL-C升高,差异有统计学意义(P<0.05);与治疗前相比,两组治疗14 d后FIB、HCT、CP均呈下降趋势,但实验组下降更为明显,差异有统计学意义(P<0.05)。结论 养心健脾汤治疗UA,能有效调节血脂,改善血液流变学,减轻临床症状,增强疗效。
Objective To investigate the therapeutic effect of Yangxin Jianpi Decoction on unstable angina pectoris(UA)and its influence on blood lipids and hemorheology.Methods A total of eighty patients with UA in our hospital(January-July 2023)were selected and divided into forty cases by random number table method.The control group was given conventional Western medicine treatment,and the experimental group was given Yangxin Jianpi Decoction on this basis.Both groups were treated for 14 days.Total effective rate,reduction rate of nitroglycerin,TCM syndrome score,blood lipid[total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C)]and hemorheological indexes[fibrinogen(FIB),erythrocyte specific volume(HCT),blood viscosity(CP)]were compared between the two groups.Results The total effective rate of experimental group was 90.00%higher than that of control group 72.50%(P<0.05).The reduction rate of nitroglycerin in experimental group was 87.50%higher than that in control group 67.50%(P<0.05).Compared with before treatment,chest tightness,chest pain,dizziness,fatigue and fatigue in both groups decreased after 14 days of treatment,and the experimental group was lower than the control group(P<0.05).Compared with control group,TG,TC and LDL-C were decreased and HDL-C was increased in experimental group after 14 days of treatment(P<0.05).Compared with before treatment,FIB,HCT and CP in both groups showed a decreasing trend after 14 days of treatment,but the decrease in experimental group was more significant(P<0.05).Conclusion Yangxin Jianpi Decoction can effectively regulate blood lipids,improve hemorheology,reduce clinical symptoms and enhance therapeutic effect in treating UA.
