摘要
目的:探讨彩色多普勒超声联合三维超声容积测量(VOCAL)技术诊断剖宫产瘢痕妊娠的临床价值。方法:选取2021年5月-2023年5月银川市妇幼保健院收治的75例疑似剖宫产瘢痕妊娠患者作为研究对象,均行彩色多普勒超声联合三维VOCAL技术检查,以宫腔镜检查结果作为“金标准”,分析彩色多普勒超声联合三维VOCAL技术检查对剖宫产瘢痕妊娠的诊断效能。根据宫腔镜检查结果,将75例受试者分为剖宫产瘢痕妊娠组与非剖宫产瘢痕妊娠组,比较两组瘢痕肌层厚度、血管形成指数(VI)、血流指数(FI)及血管血流指数(VFI)。结果:宫腔镜检查显示,75例受试者中剖宫产瘢痕妊娠患者62例。彩色多普勒超声联合三维VOCAL技术诊断剖宫产瘢痕妊娠的灵敏度、准确度高于彩色多普勒超声检查,差异有统计学意义(P<0.05);彩色多普勒超声检查与彩色多普勒超声联合三维VOCAL技术诊断剖宫产瘢痕妊娠的特异度、阳性预测值及阴性预测值比较,差异无统计学意义(P>0.05)。剖宫产瘢痕妊娠组瘢痕肌层厚度小于非剖宫产瘢痕妊娠组,VI、FI、VFI高于非剖宫产瘢痕妊娠组,差异有统计学意义(P<0.001)。结论:彩色多普勒超声联合三维VOCAL技术诊断剖宫产瘢痕妊娠的价值较高,能够提供血流定量参数,提高诊断准确度与灵敏度。
Objective:To explore the clinical value of color Doppler ultrasound combined with three-dimensional volume measurement(VOCAL)technique in diagnosing cesarean scar pregnancy.Methods:A total of 75 patients with suspected cesarean scar pregnancy admitted to Yinchuan Women and Children Healthcare Hospital from May 2021 to May 2023 were selected as the study subjects.All the patients underwent the examination of color Doppler ultrasound combined with three-dimensional VOCAL technique.The results of hysteroscopy were used as the"gold standard"to analyze the diagnostic efficiency of color Doppler ultrasound combined with three-dimensional VOCAL technique for cesarean scar pregnancy.According to the results of hysteroscopy,75 subjects were divided into cesarean scar pregnancy group and non-cesarean scar pregnancy group.The cicatricial muscle thickness,vascularization index(VI),blood flow index(FI)and vascular blood flow index(VFI)were compared between the two groups.Results:Hysteroscopy showed that 62 of the 75 subjects had cesarean scar pregnancy.The sensitivity and accuracy of color Doppler ultrasound combined with three-dimensional VOCAL technique in the diagnosis of cesarean scar pregnancy were higher than those of color Doppler ultrasound,and the difference was statistically significant(P<0.05).There was no significant difference in the specificity,positive predictive value and negative predictive value between color Doppler ultrasound and color Doppler ultrasound combined with three-dimensional VOCAL technique in the diagnosis of cesarean scar pregnancy(P>0.05).The cicatricial muscle thickness in the cesarean scar pregnancy group was lower than that in the non-cesarean scar pregnancy group,and the VI,FI and VFI in the cesarean scar pregnancy group were higher than those in the non-cesarean scar pregnancy group,and the difference was statistically significant(P<0.001).Conclusion:Color Doppler ultrasound combined with three-dimensional VOCAL technique has high value in diagnosing cesarean scar pregnancy,and can provide quantitative parameters of blood flow and improve the diagnostic accuracy and sensitivity.
