真武汤对糖尿病肾病大鼠肾功能、蛋白尿和血液流变学的影响

Effect of Zhenwu Decoction on renal function,proteinuria and hemorheology in diabetic nephropathy rats

摘要 目的分析真武汤对糖尿病肾病大鼠肾功能、蛋白尿和血液流变学的影响,及其可能潜在机制。方法24只雄性SD大鼠被分为正常组(8只)和模型组(16只),模型组通过高糖高脂喂养和腹膜内注射链脲菌素(STZ)(65 mg/kg)构建糖尿病肾病模型,构建成功后的模型组大鼠再分为模型对照组(8只,灌胃给予10 ml生理盐水)和真武汤组[8只,灌胃给药5.0 g/(kg·d)、10 ml/kg]。实验第27周,采集三组大鼠的血液和24 h尿液样本进行生化检查,测定血液流变学指标,并用逆转录聚合酶链式反应(RT-PCR)分析肾组织的nephrin和podocin水平。结果(1)实验第27周,正常组的空腹血糖(FBG)、24 h尿蛋白定量、血尿素氮(BUN)、血清肌酐(Cr)水平分别为(6.42±0.83)mmol/L、(5.12±0.76)mg/24 h、(8.41±0.42)mmol/L、(104.48±18.72)μmol/L;模型对照组分别为(21.24±2.22)mmol/L、(27.23±4.10)mg/24 h、(26.27±1.74)mmol/L、(311.91±19.94)μmol/L;真武汤组分别为(20.22±2.19)mmol/L、(10.12±1.87)mg/24 h、(12.94±2.02)mmol/L、(194.95±17.42)μmol/L。三组大鼠的FBG、24 h尿蛋白定量、BUN、Cr比较差异具有统计学意义(P<0.05);与正常组比较,模型对照组及真武汤组的FBG、24 h尿蛋白定量、BUN、Cr均明显升高(P<0.05);与模型对照组比较,真武汤组的24 h尿蛋白定量、BUN、Cr均明显降低(P<0.05)。(2)实验第27周,正常组的红细胞压积、红细胞计数、红细胞聚集指数、红细胞变形指数分别为(0.48±0.05)L/L、(5.33±0.31)×1012/L、(8.10±0.75)、(0.95±0.08);模型对照组分别为(0.57±0.03)L/L、(5.89±0.34)×1012/L、(11.20±1.96)、(0.84±0.05);真武汤组分别为(0.52±0.01)L/L、(5.60±0.15)×1012/L、(9.94±1.45)、(0.93±0.04)。三组大鼠的红细胞压积、红细胞计数、红细胞聚集指数、红细胞变形指数比较差异具有统计学意义(P<0.05);与正常组比较,模型对照组及真武汤组的红细胞压积、红细胞计数、红细胞聚集指数均明显升高,模型对照组的红细胞变形指数明显降低(P<0.05);与模型对照组比较,真武汤组的红细胞压积、红细胞计数、红细胞聚集指数均明显降低,红细胞变形指数明显升高(P<0.05)。(3)实验第27周,正常组的nephrin、podocin表达水平分别为(1.80±0.09)、(1.94±0.08);模型对照组分别为(1.00±0.07)、(0.99±0.05);真武汤组分别为(1.54±0.06)、(1.61±0.07)。三组大鼠的nephrin、podocin表达水平比较差异具有统计学意义(P<0.05);与正常组比较,模型对照组及真武汤组的nephrin、podocin均明显降低(P<0.05);与模型对照组比较,真武汤组的nephrin、podocin均明显升高(P<0.05)。结论真武汤可通过改善代谢紊乱、增加nephrin和podocin基因表达,有效减轻糖尿病肾病大鼠的肾损伤,改善肾功能,真武汤可能是治疗糖尿病肾病的有效途径。 Objective To analyze the effect of Zhenwu Decoction on renal function,proteinuria and hemorheology in diabetic nephropathy rats and its potential mechanism.Methods 24 male SD rats were divided into normal group(8 rats)and model group(16 rats).The model group was constructed as a diabetic nephropathy model by high-sugar and high-fat feeding and intraperitoneal injection of streptozotocin(STZ)(65 mg/kg),and the rats in the model group after the successful construction were subdivided into the model-control group(8 rats,10 ml of normal saline was given by gavage)and the Zhenwu Decoction group[8 rats,5.0 g/(kg·d),10 ml/kg].At the 27th week of the experiment,blood and 24 h urine samples of the three groups of rats were collected for biochemical examination,hemorheological indexes were determined,and the levels of nephrin and podocin in renal tissues were analyzed by reverse transcription polymerase chain reaction(RT-PCR).Results(i)At the 27th week of the experiment,the levels of fasting blood glucose(FBG),24-h urinary protein quantity,blood urea nitrogen(BUN)and serum creatinine(Cr)in the normal group were(6.42±0.83)mmol/L,(5.12±0.76)mg/24 h,(8.41±0.42)mmol/L and(104.48±18.72)μmol/L;those in the control group were(21.24±2.22)mmol/L,(27.23±4.10)mg/24 h,(26.27±1.74)mmol/L and(311.91±19.94)μmol/L;those in Zhenwu Decoction group were(20.22±2.19)mmol/L,(10.12±1.87)mg/24 h,(12.94±2.02)mmol/L and(194.95±17.42)μmol/L.There were significant differences in FBG,24 h urinary protein quantity,BUN and Cr among the three groups(P<0.05).Compared with normal group,FBG,24 h urinary protein quantity,BUN and Cr in model-control group and Zhenwu Decotion group were significantly increased(P<0.05).Compared with model-control group,the 24 h urinary protein quantity,BUN and Cr in Zhenwu Decoction group were significantly decreased(P<0.05).(ii)At the 27th week of the experiment,the hematocrit,erythrocyte count,erythrocyte aggregation index and erythrocyte deformation index in the normal group were(0.48±0.05)L/L,(5.33±0.31)×1012/L,(8.10±0.75)and(0.95±0.08);those in the control group were(0.57±0.03)L/L,(5.89±0.34)×1012/L,(11.20±1.96)and(0.84±0.05);those in the Zhenwu Decoction group were(0.52±0.01)L/L,(5.60±0.15)×1012/L,(9.94±1.45)and(0.93±0.04).There were significant differences in hematocrit,erythrocyte count,erythrocyte aggregation index and erythrocyte deformation index among the three groups(P<0.05).Compared with normal group,the hematocrit,erythrocyte count and erythrocyte aggregation index of model-control group and Zhenwu Decoction group were significantly increased,while the erythrocyte deformation index of model-control group was significantly decreased(P<0.05).Compared with the model-control group,the hematocrit,erythrocyte count and erythrocyte aggregation index of Zhenwu Decoction group were significantly decreased,and the erythrocyte deformation index was significantly increased(P<0.05).(iii)At the 27th week of the experiment,the expression levels of nephrin and podocin in the normal group were(1.80±0.09)and(1.94±0.08),those in the control group were(1.00±0.07)and(0.99±0.05),and those in the Zhenwu Decoction group were(1.54±0.06)and(1.61±0.07).The expression levels of nephrin and podocin in three groups were significantly different(P<0.05).Compared with normal group,the nephrin and podocin in model-control group and Zhenwu Decoction group were significantly decreased(P<0.05).Compared with model-control group,nephrin and podocin in Zhenwu Decoction group were significantly increased(P<0.05).Conclusion Zhenwu Decoction can effectively reduce kidney injury and improve renal function in rats with diabetic nephropathy by improving metabolic disorders,increasing nephrin and podocin gene expression.Zhenwu Decoction may be an effective way to treat diabetic nephropathy.
