摘要
目的观察2型糖尿病合并良性甲状腺结节男性患者的内分泌激素特征情况。方法选取2020年11月—2022年11月福建省莆田学院附属医院内分泌科收治的108例2型糖尿病合并良性甲状腺结节男性患者为观察组,并选取同期108例男性良性甲状腺结节患者为对照组。在合并2型糖尿病的患者中,以糖尿病严重程度作为依据,进一步分为轻度组(n=42)、中度组(n=36)、重度组(n=30)。比较各组血糖指标和激素水平。结果观察组空腹胰岛素、胰岛素稳态模型评估-胰岛素抵抗指数、空腹血糖、糖化血红蛋白高于对照组,差异有统计学意义(P<0.05)。观察组患者促甲状腺激素、黄体生成素指标高于对照组,差异有统计学意义(P<0.05)。两组患者血清游离甲状腺素、血清游离三碘甲腺原氨酸、性激素结合球蛋白、雌二醇、催乳素、卵泡雌激素比较,差异无统计学意义(P>0.05)。轻度组、中度组、重度组患者促甲状腺激素、黄体生成素等指标相比较,差异有统计学意义(P<0.05)。轻度组、中度组、重度组患者血清游离甲状腺素、血清游离三碘甲腺原氨酸、性激素结合球蛋白、雌二醇、催乳素、卵泡雌激素相比较,差异无统计学意义(P>0.05)。结论2型糖尿病合并良性甲状腺结节男性患者与健康人群相比血糖指标更高,激素水平呈现明显差异。糖尿病严重程度不同,激素水平同样呈现一定差异。
Objective To observe the characteristics of endocrine hormone in male patients with type 2 diabetes mellitus complicated with benign thyroid nodules.Methods A total of 108 male patients with type 2 diabetes mellitus combined with benign thyroid nodule admitted to the department of endocrinology of the Affiliated Hospital of Fujian Putian University,from November 2020 to November 2022 were selected as the observation group.A total of 108 male patients with benign thyroid nodules were selected as control group.Patients with type 2 diabetes were further divided into mild group(n=42),moderate group(n=36)and severe group(n=30)based on the severity of diabetes.Blood glucose indexes and hormone levels were compared among all groups.Results Fasting insulin,insulin homeostasis model assessment-insulin resistance index,fasting plasma glucose,glycosylated hemoglobin in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).The indexes of thyroid stimulating hormone and luteinizing hormone in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).Serum free thyroxine,serum free triiodothyronine,sex hormone binding globulin,estradiol,prolactin,follicle estrogen were compared between the two groups,the differences were not statistically significant(P>0.05).Mild group,moderate group,severe group of patients with thyroid stimulating hormone,luteinizing hormone compared,the difference was statistically significant(P<0.05).Comparison of serum free thyroxine,serum free triiodothyronine,sex hormone binding globulin,estradiol,prolactin and follicular estrogen in mild group,moderate group and severe group,the differences were not statistically significant(P>0.05).Conclusion Male patients with type 2 diabetes combined with benign thyroid nodules have higher blood glucose indexes and significantly different hormone levels compared with healthy people.The severity of diabetes was different,and the hormone levels were also different.
