摘要
目的:探讨MRI弥散加权成像及缺氧诱导因子(hypoxia-inducible factor-1,HIF-1)在新生儿缺氧缺血性脑病(hypoxic-ischemic encephalopathy,HIE)中的特征及表达。方法:回顾性分析2018年1月—2022年12月咸宁市第一人民医院收治的64例HIE新生儿的临床资料。根据病情严重程度将患儿分为轻度组(23例),中度组(24例),重度组(17例)。三组均进行MRI检测,并检测HIF-1的水平。分析MRI检测结果和轻度、中度及重度患儿典型病例图像。比较不同严重程度及不同病变类型患儿HIF-1水平。结果:MRI检出率为98.44%,漏诊1例为轻度病例。重度组、中度组HIF-1水平高于轻度组,重度组HIF-1水平高于中度组,差异有统计学意义(P<0.05);合并基底节受损、合并颅内出血患儿HIF-1水平明显高于单纯脑水肿患儿,合并颅内出血患儿HIF-1水平明显高于合并基底节损伤患儿,差异有统计学意义(P<0.05)。结论:MRI弥散加权成像对新生儿HIE检出率较高,HIE严重程度的不断加重,随之血清HIF-1表达水平也不断升高。
Objective:To investigate the characteristics and expression of MRI diffusion weighted imaging and hypoxia-inducible factor-1(HIF-1)in neonates with hypoxic-ischemic encephalopathy(HIE).Method:The clinical data of 64 neonates with HIE admitted to the Xianning First People's Hospital from January 2018 to December 2022 were retrospectively analyzed.According to the severity of the disease,the children were divided into mild group(23 cases),moderate group(24 cases)and severe group(17 cases).MRI was performed in all three groups and HIF-1 levels were measured.MRI results and typical cases images of mild,moderate and severe children were analyzed.The levels of HIF-1 in children with different severity and different lesion types were compared.Result:The detection rate of MRI was 98.44%,and one case of missed diagnosis was a mild case.The level of HIF-1 in severe group and moderate group was higher than that in mild group,and the level of HIF-1 in severe group was higher than that in moderate group,the differences were statistically significant(P<0.05).The level of HIF-1 in children with basal ganglia injury and intracranial hemorrhage was significantly higher than that in children with simple brain edema,and the level of HIF-1 in children with intracranial hemorrhage was significantly higher than that in children with basal ganglia injury,the differences were statistically significant(P<0.05).Conclusion:MRI diffusion weighted imaging has a higher detection rate of neonates with HIE,and the HIE grade increases continuously,along with the serum HIF-1 expression level.
