血清CA125、AFP、CA19-9、CEA联合检测在早期子宫颈癌诊断中的应用价值

摘要 目的:探究血清糖类抗原125(CA125)、甲胎蛋白(AFP)、糖类抗原19-9(CA19-9)、癌胚抗原(CEA)联合检测在早期子宫颈癌诊断中的应用价值。方法:选择2015年6月—2020年6月于单县中心医院收治的200例早期子宫颈癌(纳入A组)、200例子宫颈上皮内瘤变患者(纳入B组)及在本院进行常规体检的200例健康女性(纳入C组)作为研究对象。比较三组血清CA125、AFP、CA19-9、CEA水平;比较早期子宫颈癌患者不同临床分期及不同病理分型的血清指标;采用logistic回归分析早期子宫颈癌的影响因素;分析血清CA125、AFP、CA19-9、CEA联合检测对早期子宫颈癌的诊断价值。结果:A组与B组血清CA125、AFP、CA19-9、CEA水平明显高于C组,A组血清CA125、AFP、CA19-9、CEA水平明显高于B组,差异有统计学意义(P<0.05)。子宫颈癌Ⅰb期、Ⅱa期患者血清CA125、AFP、CA19-9、CEA水平明显高于Ⅰa期,Ⅱa期患者明显高于Ⅰb期,腺癌、腺鳞癌患者明显高于鳞癌患者,差异有统计学意义(P<0.05)。单一血清CA19-9诊断早期子宫颈癌的曲线下面积值(AUC)最高,以38.85 U/mL为cut-off,其诊断敏感度、特异度分别为71.00%、85.50%;联合诊断AUC为0.949,诊断敏感度、特异度分别为88.50%、89.25%。结论:血清CA125、AFP、CA19-9、CEA诊断早期子宫颈癌均可发挥良好的诊断效能,其中CA19-9单一诊断效能最佳,联合或可进一步提升诊断效能。 Objective:To analyze the application value of joint detection of serum carbohydrate antigen 125(CA125),alpha-fetoprotein(AFP),carbohydrate antigen 19-9(CA19-9)and carcinoembryonic antigen(CEA)in the diagnosis of early cervical cancer.Method:A total of 200 patients with early cervical cancer(group A)and 200 patients with cervical intraepithelial neoplasia(group B)who were admitted to Shan County Central Hospital from June 2015 to June 2020,and 200 healthy women(group C)who received routine physical examinations in the hospital during the same period were selected as the study subjects.Serum levels of CA125,AFP,CA19-9 and CEA were compared among the three groups.The serum indexes of different clinical stages and pathological types of early cervical cancer were compared.logistic regression was used to analyze the influencing factors of early cervical cancer.The diagnostic value of serum CA125,AFP,CA19-9 and CEA combined detection in early cervical cancer were analyzed.Result:The serum levels of CA125,AFP,CA19-9 and CEA in group A and group B were significantly higher than those in group C,and the serum levels of CA125,AFP,CA19-9 and CEA in group A were significantly higher than those in group B,the differences were statistically significant(P<0.05).Serum levels of CA125,AFP,CA19-9 and CEA in cervical cancer patients with stageⅠb and stageⅡa were significantly higher than those in stageⅠa,those in stageⅡa were significantly higher than those in stageⅠb,and those in adenocarcinoma and adenosquamous carcinoma were significantly higher than those in squamous carcinoma,the differences were statistically significant(P<0.05).The area under the curve(AUC)value of single serum CA19-9 in the diagnosis of early cervical cancer was the highest,with 38.85 U/mL cut-off,and the diagnostic sensitivity and specificity were 71.00%and 85.50%,respectively.Combined diagnosis AUC was 0.949,diagnostic sensitivity and specificity were 88.50%and 89.25%,respectively.Conclusion:All of serum CA125,AFP,CA19-9 and CEA can be used to diagnose early cervical cancer.Among them,CA19-9 can achieve the best performance in diagnosis with single index,joint detection of these indicators may further enhance the diagnostic performance.
