摘要
目的:探究血管内超声在冠脉支架植入术中的应用效果。方法:选取2021年1月—2022年8月厦门大学附属第一医院收治的210例拟进行冠脉支架植入术患者。根据随机数表法将其分为对照组和观察组,各105例。对照组在冠脉支架植入术中应用冠脉造影,观察组在冠脉支架植入术中应用血管内超声。比较两组支架植入合格率、1年内再狭窄率,治疗后1年内不良心血管事件发生率,治疗前及治疗后3周、3个月心肌造影参数,治疗前及治疗后12个月血管内超声检查结果。结果:对照组105例患者中支架植入82例,植入率为78.10%,观察组105例患者中支架植入98例,植入率为93.33%;观察组1年内再狭窄率显著低于对照组,差异有统计学意义(P<0.05)。观察组不良心血管事件发生率显著低于对照组,差异有统计学意义(P<0.05)。治疗后3周及3个月,观察组平台期峰值强度(A)、曲线斜率(β)及心肌血流量(A·β)均显著高于对照组,差异有统计学意义(P<0.05)。治疗后12个月,观察组最小管腔直径(MLD)显著高于对照组,病变血管狭窄率(DS)及斑块负荷(PB)显著低于对照组,差异有统计学意义(P<0.05)。结论:血管内超声在冠脉支架植入术患者中的应用效果较好,安全性较高,可显著改善患者的心肌血供。
Objective:To investigate the application effect of intravascular ultrasound in coronary stent implantation operation.Method:A total of 210 patients planned to undergo coronary stent implantation operation admitted to the First Affiliated Hospital of Xiamen University from January 2021 to August 2022 were selected.They were divided into control group and observation group according to random number table method,with 105 cases in each group.The control group used coronary angiography during coronary stent implantation operation,while the observation group used intravascular ultrasound during coronary stent implantation operation.The qualified rate of stent implantation,the rate of restenosis within 1 year,the incidence of adverse cardiovascular events within 1 year after treatment,myocardial angiographic parameters before treatment and 3 weeks and 3 months after treatment,and the examination results of intravascular ultrasound before treatment and 12 months after treatment were compared between the two groups.Result:Among the 105 patients in the control group,82 cases had stent implantation,the implantation rate was 78.10%;among the 105 patients in the observation group,98 cases had stent implantation,the implantation rate was 93.33%.The restenosis rate within 1 year in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).The incidence of adverse cardiovascular events in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).At 3 weeks and 3 months after treatment,plateau peak intensity(A),curve slope(β)and myocardial blood flow(A·β)in observation group were significantly higher than those in control group,and the differences were statistically significant(P<0.05).At 12 months after treatment,the minimum lumen diameter(MLD)in observation group was significantly higher than that in control group,and the rate of pathological vessel stenosis(DS)and plaque burden(PB)in observation group were significantly lower than those in control group,and the differences were statistically significant(P<0.05).Conclusion:The application of intravascular ultrasound in patients with coronary stent implantation has good effect and high safety,and can significantly improve the myocardial blood supply of patients.
