摘要
目的:分析山东省某三甲医院2018~2022年肺癌患者住院费用的构成情况,探讨肺癌患者住院费用的影响因素,为推进医疗费用管控工作提供参考。方法:在山东省某三级甲等医院胸外科住院患者管理系统中,选择2018年至2022年住院的肺癌患者信息资料,运用单因素和多元线性回归统计学方法综合分析影响肺癌患者住院费用的因素。结果:2018年至2022年肺癌患者的平均住院费用整体呈上升趋势。在肺癌患者住院费用的构成中,药品费、诊查费、检查费、治疗费和材料费远高于其他类型项目的费用,五项费用的结构贡献率总和为84.88%,是构成住院总费用的主要项目。影响肺癌患者住院费用的因素包括:年龄、职业、付费方式、病理类型、是否转移、肿瘤分期、治疗方式、住院天数,其中住院天数是影响住院费用最主要的因素。结论:控制药品费、材料费、缩短肺癌患者的住院天数是降低住院费用的有效途径,需进一步加强对药品及耗材在生产、流通、使用等各个环节的监管,减少患者住院期间的药品和耗材费;重视并积极开展早期筛查工作,积极推进支付方式改革,从而降低医疗费用,提高医疗服务效率。
Objective:To analyze the composition of hospitalization expenses of lung cancer patients from 2018 to 2022 in a certain grade A tertiary hospital in Shandong Province,and to explore the influencing factors of hospitalization expenses of lung cancer pa-tients,so as to provide theoretical basis for promoting DRGs paid medical work.Methods:Data on lung cancer patients admitted to a grade A tertiary hospital thoracic surgery department in Shandong from 2018 to 2022 were collected,single factor and multiple linear regression were used to analyze the factors influencing the hospitalization expenses of lung cancer patients.Results:From 2018 to 2022,The average hospitalization expenses of lung cancer patients showed an increasing trend.The cost of medicine,examination,ex-amination,treatment and material is much higher than that of other kinds of items.The total contribution rate of the five items is 84.88%,It is the main component of the total cost of hospitalization.The factors influencing the hospitalization expenses of patients with lung cancer included age,occupation,mode of payment,pathological type,metastasis,tumor stage,treatment mode and length of stay,the length of stay is the most important factor affecting the cost of hospitalization.Conclusion:It is an effective way to reduce the hospitalization expenses by controlling the drug expenses,material expenses and shortening the hospitalization days of lung cancer pa-tients,It is necessary to strengthen the supervision of drugs and consumables in the process of production,circulation and use,so as to reduce the drug and consumables cost during hospitalization.Pay attention to and actively carry out early screening work,actively pro-mote the reform of payment,so as to reduce medical costs,improve the efficiency of medical services.
