摘要
目的:探讨以药物咨询为切入点提升药学服务水平。方法:回顾性分析我院2018-2021年门诊药房的药物咨询记录。结果:我院2018-2021年共记录药物咨询661例次,咨询人员以患者或患者家属为主(72.92%),其次为医护人员;咨询形式以窗口咨询为主(71.10%)。咨询内容可分为10类,前三位咨询内容依次为药物的用法用量(25.57%)、药物比较与选择(15.13%)、药物使用注意事项(12.10%)。结论:药物咨询中还存在咨询问题分类缺乏统一标准、非专业性或者对药师专业提升价值不大的咨询记录多、被动药物咨询、药物咨询记录没有得到充分利用等问题。规范药物咨询问题分类,优化药物咨询记录,不断提升药物咨询记录的专业价值,多维度利用好药物咨询记录,主动提供用药指导、教育,可以提升门诊药学服务水平。
Objective:To improve the level of pharmaceutical care with drug consultation as the entry point.Methods:Retrospective analysis was carried out on the consultation records of outpatient pharmacy in our hospital from 2018 to 2021.Results:From 2018 to 2021,661 cases of drug consultation were recorded in our hospital.Most of the counselors were patients or their family members(72.92%),followed by medical staff.The form of consultation was mainly window consultation(71.10%).The consultation contents could be divided into 10 categories,the top three consultation contents were the usage and dosage of drugs(25.57%),drug comparison and selection(15.13%),and precautions for drug use(12.10%).Conclusion:There are still problems in drug counseling,such as a lack of unified classification standards for consultation issues,a large number of non professional or low value consultation records for pharmacist professional improvement,passive drug counseling,and insufficient utilization of drug counseling records.Standardizing the classification of medication consultation issues,optimizing medication consultation records,continuously improving the professional value of medication consultation records,making full use of medication consultation records from multiple dimensions,actively providing medication guidance and education can improve the level of outpatient pharmaceutical care.
