特发性肺纤维化患者血清FSTL1、Syndecan-1与肺功能和预后的关系

Study on the relationship between serum FSTL1,Syndecan-1 and lung function,prognosis in patients with idiopathic pulmonary fibrosis

摘要 目的探讨特发性肺纤维化(IPF)患者血清卵泡抑素样蛋白-1(FSTL1)、多配体蛋白聚糖-1(Syndecan-1)与肺功能和预后的关系。方法选取2021年1月至2022年1月西安国际医学中心医院收治的94例IPF患者纳入观察组,另选取同期在该院进行健康体检的健康受试者50例为对照组。比较观察组和对照组FSTL1、Syndecan-1及肺功能指标第1秒用力呼气容积(FEV1)、FEV1与用力肺活量(FVC)比值(FEV1/FVC)、FEV1占预计值百分比(FEV1%pred)水平,分析FSTL1、Syndecan-1水平与肺功能指标的相关性。观察组出院后随访1年,根据随访结果分为预后良好组和预后不良组,比较两组FSTL1、Syndecan-1水平及临床资料,并采用多因素Logistic回归分析IPF预后不良的危险因素。结果观察组FSTL1、Syndecan-1水平高于对照组,FEV1、FEV1/FVC、FEV1%pred水平低于对照组,差异有统计学意义(P<0.05)。IPF患者FSTL1、Syndecan-1水平与肺功能指标FEV1、FEV1/FVC、FEV1%pred呈负相关(P<0.05)。经1年随访观察,预后不良组(45例)FSTL1、Syndecan-1水平高于预后良好组(49例),预后不良组年龄大于预后良好组,FEV1、FEV1/FVC、FEV1%pred水平低于预后良好组,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,年龄≥64岁、FSTL1≥56 ng/mL、Syndecan-1≥87 ng/mL是IPF患者预后的独立危险因素(P<0.05,OR>1),FEV1≥1.40 L、FEV1/FVC≥60%、FEV1%pred≥66%是IPF患者预后的保护因素(P<0.05,OR<1)。结论FSTL1、Syndecan-1在IPF患者中呈高表达,并与患者的肺功能呈负相关,年龄≥64岁、FSTL1≥56 ng/mL、Syndecan-1≥87 ng/mL是IPF患者预后的独立危险因素,FEV1≥1.40 L、FEV1/FVC≥60%、FEV1%pred≥66%是IPF患者预后的保护因素。 Objective To investigate the relationship of serum follistatin-like protein 1(FSTL1)and Syndecan-1 with pulmonary function and prognosis in patients with idiopathic pulmonary fibrosis(IPF).Methods A total of 94 patients with IPF admitted to Xi′an International Medical Center Hospital from January 2021 to January 2022 were selected as the observation group,and 50 healthy subjects who underwent physical examination in the hospital during the same period were selected as the control group.The levels of FSTL1,Syndecan-1 and pulmonary function indexes such as forced expiratory volume in one second(FEV1),FEV1 to forced vital capacity(FVC)ratio(FEV1/FVC)and FEV1 percent predicted value(FEV1%pred)were compared between the observation group and the control group.The correlation between FSTL1,Syndecan-1 levels and pulmonary function indexes was analyzed.The observation group was followed up for 1 year after discharge,and was divided into good prognosis group and poor prognosis group according to the follow-up results.The levels of FSTL1 and Syndecan-1,clinical data were compared between the two groups,and multivariate Logistic regression was used to analyze the risk factors for poor prognosis of IPF.Results The levels of FSTL1 and Syndecan-1 in the observation group were higher than those in the control group,and the levels of FEV1,FEV1/FVC and FEV1% pred were lower than those in the control group,the differences were statistically significant(P<0.05).In IPF patients,FSTL1 and Syndecan-1 levels correlated negatively with FEV1,FEV1/FVC,and FEV1% pred(P<0.05).After one-year follow-up observation,the levels of FSTL1 and Syndecan-1 in the poor prognosis group(45 cases)were higher than those in the good prognosis group(49 cases).The age was older than that of the good prognosis group,and the levels of FEV1,FEV1/FVC and FEV1%pred were lower than those of the good prognosis group,the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that age≥64 years old,FSTL1≥56 ng/mL and Syndecan-1≥87 ng/mL were independent risk factors for prognosis in patients with IPF(P<0.05,OR>1).FEV1≥1.40 L,FEV1/FVC≥60% and FEV1% pred≥66% were protective factors for the prognosis of patients with IPF(P<0.05,OR<1).Conclusion FSTL1 and Syndecan-1 expressed highly in patients with IPF,and correlated negatively with lung function.Age≥64 years,FSTL1≥56 ng/mL and Syndecan-1≥87 ng/mL are independent risk factors for prognosis in patients with IPF.FEV1≥1.40 L,FEV1/FVC≥60%,FEV1% pred≥66% are protective factors for prognosis of patients with IPF.
