摘要
目的分析2020-2022年中国中医科学院广安门医院南区中成药基药使用情况,发现和解决该院在基药中成药品种配备和使用方面的问题,促进临床合理使用中成药国家基药。方法应用Excel统计基药中成药的品种、使用金额、金额占比、用药频度DDDs。结果中成药在有些病位常见病种证型无分布;每年基药及中成药基药使用金额门诊明显高于病区,中成药使用金额中中成药基药使用占比从27.0%下降至20.5%;连续三年医院DDDs使用前10名中成药基药品种主要是心脑血管类;中成药基药使用金额最多的是内分泌科;中成药使用中基药占比最高的科室是肿瘤科。结论该院基药使用未达标,中成药使用金额占总使用金额的55%-60%,而基药中成药的使用连续三年下降,最高占25.2%,分析其原因主要有粗放管理及不合理使用等四个方面,调整该院品种结构也存在国家基药品种较少、证型分类不全等三个难点。期待新版国家基药目录的出台,结合该院临床实际,以对国家基药实施科学、有效地合理使用和监测。
Objective Analze the use of Chinese patent drug which are national essential medicines to find some insufficient about variety configuration and practice from 2020-2022.Method Excel was used to count the varieties,amount of use,proportion of amount,DDDs of base drug traditional Chinese patent medicines and simple preparations.Results Traditional Chinese patent medicines and simple preparations has no distribution in some common diseases;the annual use amount of basic drugs and traditional Chinese patent medicines and simple preparations in outpatient department is significantly higher than that in ward,and the use proportion of traditional Chinese patent medicines and simple preparations in the use amount of traditional Chinese patent medicines and simple preparations decreases from 27.0%to 20.5%.The top 10 DDDs are mainly cardiovascular and cerebrovascular drugs;the largest amount of money is used at endocrinology department,and the highest proportion are used at oncology department.Conclusion The require for national essential medicines is not up to the standard.The amount of Chinese patent drug accounts for 55%-60%of the total,while Chinese patent essential medicines declined for three consecutive years,the higest is 25.2%.The reasons mainly includes four aspects,such as unreasonable use.There are also three difficulties in adjusting the variety structure of medicines such as fewer national essential drugs and incomplete classification of syndrome types and so on.
