摘要
目的:探究逆行肾内输尿管软镜碎石术(RIRS)对肾结石患者机体氧化应激反应的影响。方法:选取2021年7月—2022年12月宜春市中医院收治的80例肾结石患者,按随机数字表法分为两组,各40例。观察组采取RIRS治疗,对照组采用微通道经皮肾镜碎石术(mPCNL)治疗。比较两组围手术期指标、一期净石率、氧化应激反应[超氧化物歧化酶(SOD)、皮质醇(Cor)、丙二醛(MDA)、谷胱甘肽过氧化物酶(GSH-Px)]、并发症发生率。结果:观察组住院时间短于对照组,一期净石率高于对照组,差异均有统计学意义(P<0.05)。术后1 d,两组MDA、Cor均较术前升高,但观察组较对照组均更低,两组SOD、GSH-Px均较术前降低,但观察组较对照组均更高,差异均有统计学意义(P<0.05)。两组手术时间和并发症总发生率比较,差异均无统计学意义(P>0.05)。结论:RIRS、mPCNL均可有效治疗肾结石患者,且安全性较高,但RIRS可缩短患者的住院时间,有效提高患者的一期净石率,缓解其氧化应激反应。
Objective:To investigate the effect of retrograde intrarenal surgery(RIRS)on oxidative stress reaction in patients with renal calculus.Method:A total of 80 patients with renal calculus admitted to Yichun Hospital of Traditional Chinese Medicine from July 2021 to December 2022 were selected and divided into two groups according to random number table method,with 40 cases in each group.The observation group was treated with RIRS,and the control group was treated with microchannel percutaneous nephrolithotripsy(mPCNL).Perioperative period indexes,primary stone purification rate,oxidative stress reaction[superoxide dismutase(SOD),cortisol(Cor),malondialdehyde(MDA),glutathione peroxidase(GSH-Px)]and complication rate were compared between the two groups.Result:The hospital stay of the observation group was shorter than that of the control group,and the primary stone purification rate of the observation group was higher than that of the control group,the differences were statistically significant(P<0.05).1 day after surgery,MDA and Cor in both groups were higher than those before surgery,but those in the observation group were lower than those in the control group,SOD and GSH-Px in both groups were lower than those before surgery,but those in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).There were no significant difference in operation time and total complication rate between the two groups(P>0.05).Conclusion:Both RIRS and mPCNL can effectively treat patients with renal calculus with high safety,but RIRS can shorten the hospital stay of patients,effectively improve the primary stone purification rate of patients,and alleviate their oxidative stress reaction.
