TICL V4c植入术后拱高对散光轴旋转角度的影响

摘要 目的 分析有晶体眼散光型人工晶体(TICl V4c)植入术后拱高对人工晶体散光轴旋转角度的影响。方法 选取眼科接受TCIL V4c植入术的近视患者89例(174眼),按照术后1周拱高的大小分为低拱高组、中拱高组以及高拱高组,术前检测患者裸眼视力(UCVA)、最佳矫正视力(BCVA)、屈光情况(球镜度数及柱镜度数)、非接触式眼压(IOP)、前房深度(ACD)以及内皮细胞计数(ECD);术后随访2年,记录患者的拱高变化情况及散光轴旋转角度情况,分析拱高与散光轴旋转角度的关系。结果 TICl V4c植入术后2年,低、中、高拱高组3组患者术后平均UCVA均高于术前(P<0.01);术后2年,3组患者的UCVA(0.97±0.24)好于术前BCVA(t=-8.78,P<0.01);3组近视患者术前的平均球镜度(-10.10±3.76)D提升至术后的(-0.39±0.81)D,术前的平均柱镜度(-2.36±1.03)D提升至术后的(-0.44±1.53)D(P<0.01);术后随访3组患者IOP、ECD均保持在正常范围内,且与术前比较,差异无统计学意义(P>0.05);所有患者术后均未发生青光眼、视网膜脱离以及葡萄膜炎等严重并发症;随访2年中3组患者的拱高变化不明显(P>0.05);患者晶体散光轴均发生不同程度的旋转,相关性分析表明,拱高与散光轴旋转角度不存在相关性(r=-0.117,P>0.05)。结论 TICL V4c植入术具有良好的有效性和安全性,术后产生的拱高对散光轴旋转角度并无明显影响。 Objective To analyze the effect of vault on the rotation angle of astigmatic axial of intraocular lens after the implantation of toric implantable collamer lens V4c(TICL V4c).Methods A total of 174 eyes from 89 myopia patients who were given with TCIL V4c implantation were selected and divided into low-,medium-and high vault groups based on the vault heights after surgery.The patients were examined for uncorrected visual acuity(UCVA),best-corrected visual acuity(BCVA),refractive errors(diopter of spherical power and diopter of cylindrical power),non-contact intraocular pressure(IOP),anterior chamber depth(ACD),and endothelial cell density(ECD)before surgery.The patients were followed up for 2 years after surgery.Vault changes,rotation angle of astigmatism axis were recorded.The correlation was analyzed between vault and rotation angle of astigmatism axis.Results At two years after TICl V4c implantation,the average postoperative UCVAs of the patients in the low-,medium-and high-vault groups were higher than those before surgery(P<0.01).At two years after surgery,UCVAs of the patients in three groups(0.97±0.24)was better than their preoperative BCVAs(t=-8.78,P<0.01).The mean preoperative diopter of spherical power(-10.10±3.76)D of the myopic patients in three groups was increased to postoperative(-0.39±0.81)D,and the mean preoperative diopter of cylindrical power(-2.36±1.03)was increased to postoperative(-0.44±1.53)D(P<0.01).IOPs and ECDs of the followed-up patients in three groups after surgery remained within the normal range,and there were no statistically significant differences between before and after surgery(P>0.05).All patients did not experience serious complications such as glaucoma,retinal detachment and uveitis after surgery.During the 2-year follow-up,there was no significant change in vault among three groups(P>0.05).The patient's astigmatic axis underwent varying degrees of rotation.Correlation analysis showed that the vault did not affect the rotation angle of the astigmatic axis(r=-0.117,P>0.05).Conclusion TICL V4c implantation has good effectiveness and safety,and surgery-induced vault has no significant impact on the rotation angle of the astigmatism axis.
