2型糖尿病患者血清SUA与IGF-1及FGF-19水平与糖尿病视网膜病变的关系 被引量:1

Correlation between serum SUA,IGF-1,and FGF-19 and diabetic retinopathy in patients with type 2 diabetes

摘要 目的分析血清尿酸(serum uric acid,SUA)与胰岛素样生长因子-1(insulin-like growth factor-1,IGF-1)及成纤维细胞生长因子-19(fibroblast growth factor-19,FGF-19)水平与2型糖尿病患者合并糖尿病视网膜病变(diabetic retinopathy,DR)的相关性。方法本研究纳入2020年5月至2022年5月漯河市第三人民医院收治的124例2型糖尿病患者,按照有无糖尿病视网膜病变分为非DR组(n=76)及DR组(n=48)。比较两组患者基本资料、血清学指标及血清SUA、IGF-1与FGF-19水平的差异。采用Pearson相关性分析探讨血清SUA、IGF-1及FGF-19水平与血清学指标的相关性,绘制受试者工作特征(ROC)曲线分析血清SUA、IGF-1及FGF-19对DR的预测价值。结果DR组平均年龄为(59.31±8.36)岁,病程为(9.22±3.01)年;非DR组平均年龄为(53.58±8.29)岁,病程为(6.71±2.54)年,DR组患者年龄及病程均高于非DR组,且差异具有统计学意义(P<0.001)。与非DR组相比,DR组患者血清FPG、HbA1c、LDL-C、TG、SUA及IGF-1水平升高,且差异具有统计学意义(P<0.05)。与非DR组相比,DR组患者血清HDL-C及FGF-19水平降低,且差异具有统计学意义(P<0.001)。血清SUA及IGF-1与FPG、HbA1c、TC及TG呈显著正相关(P<0.05);血清FGF-19水平与FPG、HbA1c、TC及TG呈显著负相关(P<0.001)。血清SUA诊断DR的AUC为0.846(95%CI=0.7787~0.9138),最佳截断值为366.8μmol/L;血清FGF-19诊断DR的AUC为0.927(95%CI=0.8747~0.9789),最佳截断值为8.661μg/mL,血清SUA及FGF-19对DR有较高的预测价值。结论血清SUA及IGF-1水平在DR患者升高,血清FGF-19在DR患者中下降。血清SUA及FGF-19对DR具有较高的预测价值。 Objective To analyze the correlation between serum uric acid(SUA),insulin-like growth factor-1(IGF-1),and fibroblast growth factor-19(FGF-19)levels and diabetic retinopathy(DR)in type 2 diabetic patients.Methods A total of 124 patients with type 2 diabetes admitted to the Third People's Hospital of Luohe City from May 2020 to May 2022 were divided into non-DR group(n=76)and DR Group(n=48)based on DR.The differences of basic data,serological indexes,and serum SUA,IGF-1,and FGF-19 levels between the two groups were compared.Pearson correlation analysis was used to investigate the correlation between serum SUA,IGF-1,and FGF-19 levels and serologi-cal indexes,and receiver operating characteristic(ROC)curve was drawn to analyze the predictive value of serum SUA,IGF-1,and FGF-19 for DR.Results The mean age of DR group was(59.31±8.36)years,and the course of disease was(9.22±3.01)years.The mean age of the non-DR group was(53.58±8.29)years,and the course of disease was(6.71±2.54)years.The age and course of disease in the DR group were higher than those of the non-DR group,and the differences were statistically significant(P<0.001).Compared with non-DR group,serum levels of FPG,HbA1c,LDL-C,TG,SUA,and IGF-1 in DR group were increased,and the differences were statistically significant(P<0.05).Compared with non-DR group,serum HDL-C and FGF-19 levels in DR group were decreased,and the differences were statistically significant(P<0.001).Serum SUA and IGF-1 levels were positively correlated with FPG,HbA1c,TC,and TG levels(P<0.05).Serum FGF-19 level was negatively correlated with FPG,HbA1c,TC,and TG(P<0.001).The AUC of serum SUA in the diagnosis of DR was 0.846(95%CI=0.7787-0.9138),and the optimal cut-off value was 366.8μmol/L.The AUC of serum FGF-19 in the diagnosis of DR was 0.927(95%CI=0.8747-0.9789),and the optimal cut-off value was 8.661μg/mL.Serum SUA and FGF-19 had high predictive values for DR.Conclusion Serum SUA and IGF-1 levels were increased in DR patients,while serum FGF-19 level was decreased in DR Patients.Serum SUA and FGF-19 had high predictive values for DR.
