卵巢癌化疗患者血清LDH、YKL-40检测对PICC导管相关性血流感染的预测价值分析

Analysis of the predictive value of serum LDH and YKL-40 detection in patients with ovarian cancer undergoing chemotherapy for PICC catheter-related infection

摘要 目的:探究卵巢癌化疗患者血清乳酸脱氢酶(LDH)、人软骨糖蛋白-39(YKL-40)检测对经外周中心静脉置管(PICC)导管相关性血流感染(CRBSI)的预测价值.方法:选取2017年5月-2021年4月我院500例卵巢癌PICC置管化疗患者作为研究对象,其中感染者50例作为感染组,另自450例非感染患者中随机抽取符合纳入排除标准的50例作为非感染组.比较两组临床资料、血清LDH、YKL-40、PCT、CRP水平,分析卵巢癌化疗患者CRBSI影响因素及血清LDH、YKL-40、PCT、CRP水平与影响因素的相关性,并评价血清LDH、YKL-40、PCT、CRP水平对卵巢癌化疗患者CRBSI的预测价值.结果:两组置管时间、化疗疗程、糖尿病史、换药频次、外周血象比较,差异有统计学意义(P<0.05);感染组血清LDH、YKL-40水平明显高于非感染组,差异有统计学意义(P<0.05);化疗疗程、外周血象、糖尿病史、置管时间、换药频次、血LDH、YKL-40、PCT、CRP水平均为卵巢癌化疗患者CRBSI的独立危险因素(P<0.05);卵巢癌化疗CRBSI患者血清LDH、YKL-40、PCT、CRP水平与置管时间、化疗疗程、糖尿病史、换药频次、外周血象呈正相关关系(P<0.05);血清LDH、YKL-40、PCT、CRP水平对卵巢癌化疗患者CRBSI的预测价值的AUC分别为0.720、0.720、0.927、0.897,应用Logistic二元回归拟合,构建各指标联合预测感染的AUC,结果显示,各指标联合的AUC为0.956.结论:卵巢癌化疗CRBSI患者血清LDH、YKL-40水平呈高表达状态,临床可监测其水平变化,预测CRBSI发生,便于临床及早进行防治工作. Objective:To investigate the correlation between serum lactate dehydrogenase(LDH),and human cartilage glycoprotein-39(YKL-40)in ovarian cancer patients undergoing chemotherapy Predictive value of sexual infections.Methods:500 patients with ovarian cancer PICC catheterization chemotherapy in our hospital from May 2017 to April 2021 were selected as the research objects,of which 50 patients were infected as the infection group,and 450 non-infected patients were randomly selected to meet the inclusion and exclusion criteria 50 cases were regarded as non-infected group.Compare the clinical data,serum levels ofLDH,and YKL-40 between the two groups,analyze the influencing factors of PICC duct-related infection in patients with ovarian cancer chemotherapy and the correlation between serum levels ofLDH,and YKL-40 and the influencing factors,and plot the receiver operating(ROC)curve,evaluate the predictive value of serumLDH and YKL-40 levels for PICC catheter-related infection in patients with ovarian cancer undergoing chemotherapy.Results:Comparing the time of catheter placement,chemotherapy course,history of diabetes,frequency of drug changes,and peripheral blood picture between the two groups,the differences were statistically significant(P<0.05);Serum LDH and YKL-40 levels were significantly higher in the infected group than in the non-infected group,and the diferences were statistically significant(P<0.05);The results oflogistic regression equation showed that the time of catheter placement,chemotherapy course,history of diabetes,frequency of drug changes,the results of logistic regression equation showed that time of placement,chemotherapy course,history of diabetes,frequency of drug changes,peripheral blood level,blood LDH,YKL-40,PCT and CRPlevels were all independent risk factors for PICC catheter-associated infection in ovarian cancer chemotherapy patients(P<0.05);Serum LDH,YKL-40,PCT and CRP levels were positively correlated with time ofplacement,chemotherapy course,history ofdiabetes,frequency ofdrug changes and peripheral blood level in ovarian cancer chemotherapy PICC catheter-associated infection patients(P<0.05).Correlation(P<0.05);The AUCs of serum LDH,YKL-40,PCT,and CRPlevels for predicting the value ofPICC catheter-associated infection in ovarian cancer chemotherapy patients were 0.720,0.720,0.927,and 0.897,respectively,and logistic binary regression was applied to fit and construct the AUCs for the combination of each indicator to predict infection.AUC for the combination of indicators was 0.956.Conclusion:The serum levels of LDH,and YKL-40 in patients with PICC catheter-related infections in ovarian cancer chemotherapy are highly expressed.The changes in their levels can be monitored clinically to predict the occurrence of PICC catheter-related infections,which is convenient for early clinical prevention and treatment.
