摘要
目的探析对宫颈机能不全患者应用经会阴超声检查与经腹部超声检查的临床诊断意义。方法纳入2020年1月至2022年1月河南科技大学第一附属医院收治的80例疑似宫颈机能不全患者,对患者同时进行经会阴超声检查、经腹部超声检查,进行术后活检确诊并作为本研究的“金标准”,比较经会阴超声、经腹部超声检查下对宫颈机能不全患者的检出率、诊断效能以及图像满意度。结果术后病理活检对患者确诊为宫颈机能不全患者46例,经会阴超声检查对收治患者宫颈机能不全检出率[73.91%(34/43)]高于经腹部超声检出率[60.87%(28/46)],差异有统计学意义(P<0.05)。诊断效能方面,经会阴超声检查的敏感性、准确性高于经腹部超声检查,差异有统计学意义(P<0.05)。经会阴超声检查的宫颈内口图像、外口图像满意度均高于经腹部超声检查,差异有统计学意义(P<0.05)。结论对宫颈机能不全患者应用经会阴超声检查在诊断疾病上有令人满意的效果,且对收治患者的健康和预后方案调整均有良好的指导价值。
Objective To explore the clinical diagnostic significance of using trans-perineum ultrasonography and transabdominal ultrasonography in patients with cervical insufficiency.Methods A total of 80 suspected patients with cervical insufficiency admitted to the First Affiliated Hospital of Henan University of Science and Technology from January 2020 to January 2022 were included in the study.Patients underwent both trans-perineal and transabdominal ultrasound examinations,Postoperative biopsy was performed to confirm the diagnosis and served as the“Gold standard”for this study.The detection rate,diagnostic efficacy,and image satisfaction of cervical insufficiency patients were compared between trans-perineal and transabdominal ultrasound examinations.Results Postoperative pathological biopsy confirmed 46 patients with cervical insufficiency.The detection rate of cervical insufficiency in admitted patients by trans-perineal ultrasound examination[73.91%(34/43)]was higher than that by transabdominal ultrasound examination[60.87%(28/46)],and the difference was statistically significant(P<0.05).In terms of diagnostic efficacy,the sensitivity and accuracy of trans-perineal ultrasound examination are higher than those of transabdominal ultrasound examination,and the difference is statistically significant(P<0.05).The satisfaction level of cervical internal and external opening images obtained through trans-perineal ultrasound examination was higher than that obtained through transabdominal ultrasound examination,and the difference was statistically significant(P<0.05).Conclusion The application of trans-perineal ultrasound in the diagnosis of cervical insufficiency patients has satisfactory results,and has good guiding value for the health and prognosis adjustment of admitted patients.
