摘要
目的探讨阿莫西林联合雷贝拉唑高剂量二联疗法(HDDP)根除幽门螺杆菌的临床效果。方法选取我院消化内科在2023年1月至2023年10月间收治的幽门螺杆菌患者,共计110例,将其通过随机数字表法分成两组。观察组55例,应用阿莫西林联合雷贝拉唑高剂量二联疗法;对照组55例,应用铋剂四联疗法。治疗结束4周后对所有患者均复查^(13)C-尿素呼气试验。对两组治疗有效性、治疗依从性、不良反应、Hp根除率。结果就治疗总有效率看,观察组为92.73%,对照组为90.91%,两组对比无明显差别,P>0.05。就治疗总依从性看,观察组为90.91%,对照组为72.73%,两组有明显差别,P<0.05。就不良反应的总发生率看,观察组为10.91%,对照组为25.45%,两组有明显差别,P<0.05。就Hp的根除率看,观察组的PP根除率为94.55%,ITT根除率为96.36%,对照组的PP根除率为74.55%,ITT根除率为72.73%,两组对比有明显差别,P<0.05。结论对于幽门螺杆菌患者,应用阿莫西林联合雷贝拉唑高剂量二联疗法的效果显著,能够提高患者的治疗有效性和治疗总依从性,并减少不良反应,提高Hp根除率。
Objective To investigate the clinical efficacy of high-dose dual therapy with amoxicillin and rabeprazole(HDDP)in eradicating Helicobacter pylori.Methods A total of 110 patients with Hp infection admitted to our department of gastroenterology between January 2023 and October 2023 were randomly divided into two groups.The observation group(n=55)received high-dose dual therapy with amoxicillin and rabeprazole,while the control group(n=55)received bismuth quadruple therapy.Four weeks after completion of treatment,all patients underwent the^(13)C-urea breath test.The efficacy of treatment,treatment compliance,adverse reactions,and Hp eradication rates were compared between the two groups.Results The overall treatment effectiveness was 92.73%in the observation group and 90.91%in the control group,with no significant difference between the two groups(P>0.05).The overall treatment compliance was 90.91%in the observation group,significantly higher than the control group's rate of 72.73%(P<0.05).The total incidence of adverse reactions was 10.91%in the observation group,significantly lower than the control group's rate of 25.45%(P<0.05).The Hp eradication rates in the observation group were 94.55%(per-protocol analysis)and 96.36%(intention-to-treat analysis),while the control group had rates of 74.55%(per-protocol analysis)and 72.73%(intention-to-treat analysis).The differences between the two groups were statistically significant(P<0.05).Conclusions High-dose dual therapy with amoxicillin and rabeprazole demonstrates significant efficacy in patients with Hp infection.It improves treatment effectiveness and compliance,reduces adverse reactions,and enhances Hp eradication rates.Therefore,it is worth considering the clinical application and promotion of this therapy for Hp eradication.
