摘要
目的 观察健脾利湿化痰祛瘀法治疗活动期甲状腺相关性眼病(TAO)临床疗效。方法 将90例TAO患者按照随机数字表法分为2组,对照组45例予注射用甲泼尼龙琥珀酸钠针静脉滴注治疗,治疗组45例予健脾利湿化痰祛瘀法治疗。疗程12周。比较2组治疗前后中医证候评分,2组治疗前后及第24周随访突眼度、临床活动度评分(CAS)、胰岛素样生长因子-1(IGF-1)、透明质酸(HA)水平,统计2组疗效及不良反应情况。结果 治疗组总有效率93.3%(42/45),对照组总有效率68.9%(31/45),治疗组临床疗效优于对照组(P<0.05)。治疗组治疗后中医证候各项评分均较本组治疗前降低(P<0.05),且治疗组治疗后均低于对照组(P<0.05),对照组治疗后畏光流泪、眼睑肿胀评分均较本组治疗前降低(P<0.05)。2组治疗后及随访时突眼度均较本组治疗前降低(P<0.05),且治疗组随访时突眼度低于对照组同期(P<0.05)。2组治疗后及随访时CAS评分均较本组治疗前降低(P<0.05),且治疗组随访时CAS评分低于对照组同期(P<0.05)。2组治疗后及随访时IGF-1、HA水平均较本组治疗前降低(P<0.05),且治疗组治疗后及随访时IGF-1、HA水平均低于对照组同期(P<0.05)。治疗组不良反应发生率低于对照组(P<0.05)。结论 健脾利湿化痰祛瘀法治疗TAO患者疗效显著,能改善眼部症状。
Objective To observe the clinical efficacy of Jianpi Lishi Huatan Quyu Formula on active thyroid associated ophthalmopathy(TAO).Methods Ninety TAO patients were randomized 1∶1 to receive either iv administered with methylprednisolone sodium succinate for injection(the control group,n=45),or Jianpi Lishi Huatan Quyu Formula(the treatment group,n=45).Treated for 12 weeks,the traditional Chinese medicine(TCM)symptom scores before and after treatment,exophthalmos,clinical activity score(CAS),insulin-like growth factor-1(IGF-1),hyaluronic acid(HA)was compared before and after treatment and the 24th-week follow-up.The curative effect and adverse reactions were assessed.Results The overall effective rate in the treatment group was better than that in the control group(93.3%[42/45]vs 68.9%[31/45],[P<0.05],respectively).After treatment,TCM symptom scores in the treatment group were significantly decreased(P<0.05),and which were significantly lower than those in the control group(P<0.05).The scores of photophobia and lachrymation,eyelid edema after treatment in the control group were significantly lower than those before treatment(P<0.05).After treatment and during follow-up,the exophthalmos,CAS score,IGF-1 and HA levels in the both groups were significantly lower than those before treatment(all P<0.05),and which in the treatment group were significantly lower than synchronous those of the control group(all P<0.05).The incidence of adverse reactions was significantly lower in the treatment group than in the control group(P<0.05).Conclusion For TAO patients,Jianpi Lishi Huatan Quyu Formula has significant curative effect,which can improve eye symptoms.
