摘要
目的探讨醋酸戈舍瑞林联合反加屈螺酮炔雌醇片(Ⅱ)辅助手术治疗对子宫内膜异位症(EMS)患者血清性激素水平和术后复发的影响。方法选取2020年1月至2022年1月于青岛市市立医院就诊的104例EMS患者作为研究对象,随机分为对照组(n=52)和联合组(n=52)。对照组采用醋酸戈舍瑞林治疗,联合组采用醋酸戈舍瑞林联合反加屈螺酮炔雌醇片(Ⅱ)治疗。比较两组患者治疗前后雌二醇(E_(2))、黄体生成素(LH)、卵泡刺激素(FSH)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平及临床疗效,随访12个月期间疾病复发与妊娠情况,以及治疗期间安全性情况。结果治疗后,与对照组比较,联合组总有效率更高(P<0.05);两组治疗后E_(2)、LH、FSH水平均下降,但联合组治疗后比对照组高(P<0.05);两组治疗后IL-6和TNF-α水平均下降(P<0.05),两组间比较,差异无统计学意义(P>0.05);随访12个月期间,与对照组比较,联合组复发率更低,妊娠率更高(P<0.05);治疗期间,与对照组比较,联合组不良反应发生率更低(P<0.05)。结论醋酸戈舍瑞林联合反加屈螺酮炔雌醇片(Ⅱ)治疗EMS,可以提升治疗效果,改善性激素水平,降低疾病复发率和副作用发生率,提高妊娠率。
Objective To explore the effects of goserelin acetate combined with drospirenone and ethinylestradiol tablet(Ⅱ)assisted surgical treatment on levels of serum sex hormones and postoperative recurrence in patients with endometriosis(EMS).Methods A total of 104 patients with EMS treated in Qingdao Municipal Hospital between January 2020 and January 2022 were selected as the research objects,and they were randomly divided into control group(n=52)and combination group(n=52).The control group was treated with goserelin acetate,and the combination group was treated with goserelin acetate combined with drospirenone and ethinylestradiol tablet(Ⅱ).The levels of estradiol(E_(2)),luteinizing hormone(LH),follicle stimulating hormone(FSH),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)before and after treatment,clinical curative effect,disease recurrence and pregnancy during 12 months of follow-up,and safety during treatment were compared between the two groups.Results After treatment,total effective rate of combination group was higher than that of control group(P<0.05).After treatment,levels of E_(2),LH and FSH in both groups decreased,which were higher in combination group than control group(P<0.05).After treatment,levels of IL-6 and TNF-αdecreased in both groups(P<0.05),but there was no significant difference between the two groups(P>0.05).Compared with control group during 12 months of follow-up,recurrence rate was lower and pregnancy rate was higher in combination group(P<0.05).During treatment,the incidence of side effects was lower in combination group than control group(P<0.05).Conclusions Goserelin acetate combined with drospirenone and ethinylestradiol tablet(Ⅱ)can improve curative effect and levels of sex hormones,reduce disease recurrence rate and incidence of side effects,and increase pregnancy rate in treatment of EMS.
