摘要
目的建立便于临床应用且诊断效能高的无创预测模型辅助诊断早期非酒精性脂肪性肝病(non-alcoholic fatty liver disease,NAFLD)。方法收集2019年6月1日至2022年6月30日在浙江省人民医院确诊的NAFLD患者和健康体检者的临床资料,随机分为训练集(n=184)和验证集(n=48)。在训练集中通过多因素Logistic回归分析方法,筛选早期NAFLD诊断的血清学指标,建立NAFLD的早期无创诊断模型。使用训练集构建诊断模型,并在验证集中进行外部验证。将诊断结果与实际情况进行对比分析,证明所建模型具有一定的可靠性和有效性。最后绘制受试者操作特征曲线,并计算曲线下面积,以此评估预测模型的准确程度。结果根据二元Logistic回归分析结果,最终纳入铁蛋白等4个预测因子,建立诊断模型,经验证集数据证实诊断准确性较好。结论基于铁蛋白等血清学指标初步建立关于早期NAFLD诊断预测模型且诊断效能良好,可用于NAFLD的早期诊断。
Objective To establish a non-invasive prediction model which is convenient for clinical application and improves diagnostic efficiency to assist the diagnosis early stage of non-alcoholic fatty liver disease(NAFLD).Methods Clinical data of patients with NAFLD and some healthy patients diagnosed by imaging in Zhejiang Provincial People’s Hospital from June 1,2019 to June 30,2022 were collected and randomly divided into a training set(n=184)and a validation set(n=48).In the training set,the serological indexes of early diagnosis of NAFLD were screened by multivariate Logistic regression analysis,and the early noninvasive diagnosis model of NAFLD was established.The diagnostic model was builted through the training set and externally verified in the verification set.To assess the prediction model's accuracy,the receiver operating characteristic curve was built and the area under the curve was constructed.Results According to the results of binary Logistic regression analysis,ferritin and other four predictors were finally included,and the diagnostic model was established according to the above factors.The empirical data confirmed that the diagnostic accuracy was good.Conclusion The prediction model of early NAFLD diagnosis was established based on serological indexes such as ferritin,which has good diagnostic efficacy and can be used for early diagnosis of NAFLD.
