摘要
目的改善部分神经介入器材在X线透视中的可视性,为神经介入手术提供便利。方法将Enterprise支架置入体模内,并以数字化X线摄影(digital radiography,DR)模式对体模进行动态投照,同步修改参数值至图像最清晰,创建用于观察特定结构细节的专属透视程式。然后选择2020年5月至2021年7月于南京市第一医院接受通用透视,但不能被清晰显示其颅内支架的神经介入治疗的患者96例,对这些患者应用专属透视,并比较两种透视的清晰度、细节观察时间、辐射平均剂量率和术者视觉舒适性。结果专属透视模式创建成功;96例患者在介入术中使用专属透视对颅内支架的显示能力全部高于通用透视;专属透视平均剂量率超过通用透视(43.27±36.29)μSv/h,但专属透视平均观察时间较通用透视缩短(6.35±3.96)s,平均累计剂量减少(1.1±1.71)μSv;手术者在专属透视下的观察舒适性明显优于通用透视。结论创建专属透视程式具有可行性,是神经介入术中观察特定结构细节的有效手段,可作为通用透视的补充。
Objective To improve the visibility of some nerve interventional devices in X-ray fluoroscopy,and to provide convenience for neurointerventional surgeries.Methods The Enterprise stent was placed into a body phantom,and dynamic imaging was performed using digital radiography(DR)mode,with simultaneous adjustment of parameter values to achieve the clearest image.A specialized fluoroscopy protocol was created to observe specific stent structural details.Subsequently,this protocol was applied to 96 patients who underwent general fluoroscopy at Nanjing First Hospital from May 2020 to July 2021,but had their intracranial stents for neurointerventional treatment not clearly display.The clarity,detail observation time,average radiation dose rate and visual comfort of the operator were compared between the two types of fluoroscopy.Results The exclusive perspective mode was constructed.In all 96 cases,the use of specialized fluoroscopy for intracranial stent visualization during the intervention procedure was superior to general fluoroscopy.The average dose rate of exclusive fluoroscopy was higher than that of general fluoroscopy and the mean±sd difference was 43.27±36.29μSv/h,and the average observation time of exclusive fluoroscopy was shorter than that of general fluoroscopy and the mean±sd difference was 6.35±3.96 sand the accumulate dose of exclusive fluoroscopy was decreased 1.1±1.71μSv.The observation comfort of operators under exclusive fluoroscopy was all better than that of general fluoroscopy.Conclusion It is feasible to create a specialized fluoroscopy program,which is an effective means to observe specific structural details in neurointerventional surgery and can be used as a complement to general fluoroscopy.
