摘要
目的探讨新生儿疼痛量表(Nips)评分联合血清C反应蛋白(CRP)、降钙素原、脑脊液白细胞计数对新生儿化脓性脑膜炎(NPM)预后的预测价值。方法选择2015年1月至2021年1月宁波大学附属人民医院收治的NPM患儿70例,以中枢神经感染程度评分标准将其分为轻症感染组45例,重症感染组25例,经治疗后预后良好的52例为预后良好组,预后不良的18例为预后不良组。比较组间Nips评分、血清CRP、降钙素原水平和脑脊液白细胞计数。分析Nips评分、血清CRP、降钙素原水平、脑脊液白细胞计数及联合检测对预后的预测价值。结果重症感染组治疗前后Nips评分、血清CRP、降钙素原水平、脑脊液白细胞计数高于轻症感染组,差异均有统计学意义(均P<0.05)。治疗后预后不良组Nips评分、血清CRP、降钙素原水平、脑脊液白细胞计数高于预后良好组,差异均有统计学意义(均P<0.05)。4项指标联合预测预后的AUC为0.901,高于各单项指标。结论Nips评分联合血清CRP、降钙素原水平及脑脊液白细胞计数有助于对NPM预后的预测。
Objective To explore the predictive value of neonatal infant pain scale(Nips)score combined with serum C-reactive protein(CRP),procalcitonin and cerebrospinal fluid white blood cell(CBFWBC)count for the prognosis of neonatal purulent meningitis(NPM).Methods Seventy newborns with purulent meningitis admitted to the Affiliated People's Hospital of Ningbo University from January 2015 to January 2021 were selected.They were divided into mild infection group of 45 cases and severe infection group of 25 cases based on severity score of central nervous system infection.After treatment,52 cases had good outcome(good outcome group)and 18 cases had poor outcome(poor outcome group).Nips score,serum CRP,procalcitonin levels,and CBFWBC count were compared between groups.The value of Nips score,serum CRP,procalcitonin levels,CBFWBC count,and combined detection for predicting prognosis was analyzed.Results The Nips score,serum CRP,procalcitonin,and CBFWBC count were significantly higher in the severe infection group than in the mild infection group before and after treatment(all P<0.05)and significantly higher in the poor outcome group than in the good outcome group(all P<0.05).The area under the curve of combined four parameters for predicting prognosis was 0.901,higher than Nips score,serum CRP,procalcitonin or CBFWBC count alone.Conclusion Nips score in combination with CRP,procalcitonin and CBFWBC count is benefit for predicting the prognosis of neonatal purulent meningitis.
