摘要
目的分析血清胱抑素C、β2-微球蛋白及同型半胱氨酸联合诊断早期2型糖尿病肾病(type 2 diabetic nephropathy,T2DN)的价值。方法回顾性分析2020年5月—2023年4月期间厦门大学附属中山医院收治的768例2型糖尿病患者的资料,按照诊断结果进行分组,其中早期T2DN组360例,单纯2型糖尿病(type 2 dia-betic nephropathy,T2DM)组408例。回顾性分析同期体验的320例无2型糖尿病的健康对象的资料,将其作为对照组。3组均接受血清胱抑素C、β2-微球蛋白及同型半胱氨酸检验。比较3组血清胱抑素C、β2-微球蛋白及同型半胱氨酸水平,以及3项指标联合检验与单独检验对于早期T2DN的诊断效能。结果早期T2DN组血清胱抑素C、β2-微球蛋白及同型半胱氨酸水平高于单纯T2DM组与对照组;单纯T2DM组血清胱抑素C、β2-微球蛋白及同型半胱氨酸水平高于对照组,差异有统计学意义(P均<0.05)。血清胱抑素C、β2-微球蛋白及同型半胱氨酸联合检验T2DN的灵敏度、特异度、准确度均高于单独检验,差异有统计学意义(P均<0.05)。结论血清胱抑素C、β2-微球蛋白、同型半胱氨酸3项指标联合检验对于早期T2DN具有较为理想的诊断效能,为临床诊疗工作提供了可靠的参考指标。
Objective To evaluate the value of cystatin C,β2-microglobulin and homocysteine in the diagnosis of early type 2 diabetic nephropathy(T2DN).Methods Data of 768 patients with type 2 diabetes admitted to Zhongshan Hospital Affiliated to Xiamen University from May 2020 to April 2023 were retrospectively analyzed,and grouped ac-cording to diagnosis results.There were 360 patients in early T2DN group and 408 patients in type 2 diabetic ne-phropathy(T2DM)group.The data of 320 healthy subjects without type 2 diabetes who experienced the same period were retrospectively analyzed and used as control group.Serum cystatin C,β2-microglobulin and homocysteine were tested in all three groups.The serum levels of cystatin C,β2-microglobulin and homocysteine were compared among the three groups,and the diagnostic efficacy of the combined test and the single test for early T2DN was compared.Results The levels of cystatin C,β2-microglobulin and homocysteine in early T2DN group were higher than those in simple T2DM group and control group,the levels of cystatin C,β2-microglobulin and homocysteine in simple T2DM group were higher than those in control group,the differences were statistically significant(all P<0.05).The sensitivity,specificity and accuracy of serum cystatin C,β2-microglobulin and homocysteine combined test for T2DN were higher than those of single test,the differences were statistically significant(all P<0.05).Conclusion The combined test of serum cystatin C,β2-microglobulin and homocysteine has ideal diagnostic efficacy for early T2DN,and pro-vides a reliable reference index for clinical diagnosis and treatment.
