摘要
目的浅析复杂肝内胆管结石应用腹腔镜左半肝联合胆道镜取石治疗的临床效果,以便为日后临床制定相应的治疗方案。方法选择2020年1月—2022年12月东莞康华医院收治的复杂肝内胆管结石患者64例进行回顾性分析,依据治疗方式的不同分为对照组与观察组,每组32例。对照组接受腹腔镜下经胆总管联合胆道镜取石治疗,观察组接受腹腔镜左半肝切除联合胆道镜取石治疗,就两组患者临床疗效、肝功能指标及并发症风险进行组间比较。结果观察组患者临床治疗总有效率明显高于对照组,术后结石残留率、结石复发率、并发症风险均明显低于对照组,差异有统计学意义(P<0.05);两组患者术后肝功能指标,差异有统计学意义(P<0.05)。结论腹腔镜左半肝切除联合胆道镜取石术对复杂肝内胆管结石有很好的疗效,术后结石残留率、结石复发率低,且有很高的安全性,值得临床应用推广。
Objective To analyze the clinical effect of laparoscopic left half liver combined with choledochoscope in the treatment of complex intrahepatic bile duct stones,so as to formulate the corresponding treatment plan for future clinic.Methods A retrospective analysis was performed on 64 patients with complex intrahepatic bile duct stones collected from Dongguan Kanghua Hospital from January 2020 to December 2022.According to different treatment methods,they were divided into control group and observation group,32 cases in each group.The control group received laparoscopic choledocholithotomy combined with choledochoscopy,and the observation group received laparoscopic left hemihepatic resection combined with choledochoscopy.The clinical efficacy,liver function indexes and complication risk of the two groups were compared between the two groups.Results The total effective rate of the observation group was significantly higher than that of the control group,and the postoperative stone residual rate,stone recurrence rate and complication risk were significantly lower than that of the control group,with statistical significance(P<0.05);The postoperative liver function index of the two groups had statistical significance(P<0.05).Conclusion Laparoscopic left hemihepatectomy combined with choledochoscopic lithotomy has a good effect on complex intrahepatic bile duct stones,with low postoperative stone residual rate and stone recurrence rate,and has high safety,which is worthy of clinical application and promotion.
