血浆肾上腺髓质素一氧化氮水平对早产儿呼吸暂停的预测效能

摘要 目的探讨血浆肾上腺髓质素(ADM)、一氧化氮(NO)水平对早产儿呼吸暂停(AOP)的预测效能。方法回顾性分析2022年1月至2023年8月本院收治的80例胎龄≤32周早产儿临床资料,按照患儿出生7 d内是否出现AOP分为AOP组(51例)及无AOP组(29例),对比2组患儿临床资料,评估血浆ADM、NO水平对患儿AOP的预测效能。结果AOP组胎龄、体质量均低于无AOP组(均P<0.05),ADM水平高于无AOP组(P<0.05),NO水平低于无AOP组(P<0.05)。多因素Logistic回归分析显示胎龄、体质量、ADM及NO水平均为AOP发生的危险因素(P<0.05)。受试者工作特征曲线(ROC)分析显示:ADM约登指数(0.730)最大时对应截断值为2278.50 pmol/L,曲线下面积为0.854,预测AOP的灵敏度、特异度分别为90.2%、82.8%;NO约登指数(0.429)最大时对应截断值为71.99μmol/L,曲线下面积为0.757,预测AOP的灵敏度、特异度分别为分别为84.3%、58.6%;二者联合时预测AOP的AUC为0.862,灵敏度、特异性分别为88.2%、82.8%。结论血浆ADM、NO水平为早产儿呼吸暂停发生的危险因素,对早产儿呼吸暂停发生的预测价值较高。 Objective To explore the predictive efficacy of plasma adrenomedullin(ADM)and nitric oxide(NO)levels on apnea of prematurity(AOP)in infants.Methods A retrospective analysis was conducted on the clinical data of 80 premature infants with gestational age≤32 weeks admitted to our hospital from January 2022 to August 2023.The infants were divided into AOP group(n=51)and non AOP group(n=29)based on whether they had AOP within 7 days of birth.The clinical data of the two groups were compared to evaluate the predictive efficacy of plasma ADM and NO levels on AOP in children.Results The gestational age and body mass of the AOP group were lower than those of the non AOP group(all P<0.05),ADM levels were higher than those of the non AOP group(P<0.05),and NO levels were lower than those of the non AOP group(P<0.05).Multivariate logistic regression analysis showed that gestational age,body mass,ADM,and NO levels were all risk factors for the occurrence of AOP(P<0.05).The receiver operating characteristic curve(ROC)analysis showed that the cutoff value corresponding to the maximum ADM Jordan index(0.730)was 2278.50 pmol/L,and the area under the curve was 0.854.The sensitivity and specificity of predicting AOP were 90.2%and 82.8%,respectively.The truncation value corresponding to the maximum NO Jordan index(0.429)is 71.99μmol/L,with an area under the curve of 0.757,the sensitivity and specificity for predicting AOP were 84.3%and 58.6%,respectively;When combined,the predicted AUC of AOP was 0.862,with sensitivity and specificity of 88.2%and 82.8%,respectively.Conclusion Plasma ADM and NO levels are risk factors for the occurrence of premature apnea,and have high predictive value for the occurrence of premature apnea.
