电针治疗卒中后吞咽障碍神经机制研究

Neural Mechanism Analysis of Electroacupuncture in the Treatment of Post-stroke Dysphagia

摘要 近年来,电针以其治疗卒中后吞咽障碍(post-stroke dysphagia,PSD)在临床上取得的良好疗效,而见诸于临床报道中。与手针相比,电针具有刺激量稳定、可重复性高且不完全依赖医者手法操作等优势,因此在临床上得以广泛应用。作者通过查阅相关文献,发现关于电针治疗卒中后吞咽障碍机制的相关研究日益丰富,但尚缺乏归纳与总结,因此制约了其进一步运用与推广。该文从神经解剖学及神经生理学等角度出发,通过整理、分析近年来相关文献,溯源求本,发现电针治疗PSD主要涉及的神经机制,包括调节、兴奋皮质吞咽中枢、皮质下结构、脑干吞咽中枢等中枢神经系统及调节吞咽功能相关的周围神经及肌群等方面。最终,试对电针治疗PSD的神经机制进行梳理总结,以期为电针治疗PSD提供可视化依据,并为未来电针的相关机制研究提供有益参考。 In recent years,electroacupuncture(EA)has been reported in clinical reports for its effectiveness in the treatment of post-stroke dysphagia(PSD).Compared with acupuncture,EA has the advantages of stable stimulation,high reproducibility and does not depend entirely on the practitioners'manipulation,and therefore is widely used in clinical practice.The author has reviewed the relevant literature and found that there is a growing body of research on the mechanisms of EA in the treatment of PSD,but there is a lack of generalization and summary,thus limiting its further use and promotion.In this paper,we have analysed the recent literature from the perspectives of neuroanatomy and neurophysiology,and found that the main neurological mechanisms involved in EA treatment of PSD include the regulation and excitation of the central nervous system such as the cortical swallowing centre,the subcortical structures,the brainstem swallowing centre,and the peripheral nerves and muscle groups related to swallowing function.Finally,this paper attempts to summarize the neural mechanisms involved in EA for the treatment of PSD,in order to provide a visual basis for EA for the treatment of PSD,and to provide useful references for future research on the mechanisms involved in EA.
