某医院耐碳青霉烯类肠杆菌科细菌分布及耐药情况分析

摘要 目的:分析河池市人民医院耐碳青霉烯类肠杆菌科细菌(CRE)的分布及耐药情况。方法:选取2020年1月—2022年12月河池市人民医院收治的97例CRE感染者作为研究对象,收集感染者临床资料,采集标本分离培养,进行细菌鉴定和药敏试验,分析CRE分布及耐药情况。结果:CRE感染者主要来自手足外科(15.46%)和泌尿外科(14.43%)。CRE标本主要为尿液标本(38.14%),其次为呼吸道标本(27.84%)。CRE以肺炎克雷伯杆菌(34.02%)为主,其次为大肠埃希菌(30.93%)与阴沟肠杆菌(12.37%)。肺炎克雷伯杆菌对亚胺培南耐药率最高,大肠埃希菌对氨苄西林、哌拉西林/他唑巴坦、头孢呋辛、阿米卡星耐药率最高,阴沟肠杆菌对氨苄西林、氨苄西林/舒巴坦、氨曲南、复方新诺明耐药率最高。结论:河池市人民医院CRE在创伤骨科、手足外科、小儿外科及泌尿外科等科室中感染率较高,主要集中在泌尿生殖道及呼吸道,细菌类型以肺炎克雷伯杆菌为主,严重多重耐药。临床需重视CRE感染者,做好病原菌鉴定和病原菌培养工作,给予最佳药物治疗。 Objective:To analyze the distribution and drug resistance of carbapenem-resistant Enterobacteriaceae(CRE) in the People's Hospital of Hechi.Methods:A total of 97 cases infected with CRE who were admitted to the People's Hospital of Hechi from January 2020 to December 2022 were selected as the study subjects.The clinical data of infected cases were collected,the specimens were collected,isolated and cultured for bacterial identification and drug susceptible tests.The distribution and drug resistance of CRE were analyzed.Results:The cases infected with CRE were mainly selected from hand and foot surgery department(15.46%) and urology surgery department(14.43%).CRE specimens were mainly urine specimens(38.14%),followed by respiratory tract specimens(27.84%).CRE was mainly Klebsiella pneumoniae(34.02%),followed by Escherichia coli(30.93%)and Enterobacter cloacae(12.37%).Klebsiella pneumoniae had the highest resistance rate to imipenem.Escherichia coli had the highest resistance rate to ampicillin,piperacillin/tazobactam,cefuroxime and amikacin.Enterobacter cloacae had the highest resistance rate to ampicillin,ampicillin/sulbactam,aztreonam,and compound sulfamethoxazole.Conclusion:CRE has a relative infection rate in the departments of trauma orthopedics,hand and foot surgery,pediatric surgery and urology surgery in the People's Hospital of Hechi,the infection mainly occurs in the genitourinary tract and respiratory tract,and the bacterial types are mainly Klebsiella pneumoniae,which has serious multi-drug resistance.In clinical practice,it is necessary to pay attention to the cases infected with CRE,perform pathogen identification and pathogen culture,and give the optimal drug treatment.
