摘要
目的 分析近3年某三级甲等精神专科医院住院青少年精神障碍患者疾病种类变化,为青少年精神障碍防治提供参考。方法 将近3年某三级甲等精神专科医院住院青少年患者设为研究组,之前3年患者设为对照组,分析其病案首页数据,总结并比较两组患者一般资料及病种构成变化。结果 共纳入患者8 889例,对照组3 584例,研究组5 305例,两组患者疾病种类大致相同,病种顺位差异不大,但研究组患者非自杀性自伤比率、首诊数量和住院消耗均高于对照组(P<0.01)。在情感性精神障碍患者的病种构成变化中,研究组患者抑郁发作占比高于对照组(P<0.01)。多因素Logistic回归分析显示,性别、童年伤害、户口类别、家族史、重大公共卫生事件均在不同程度上是两组患者精神障碍疾病比率变化的影响因素(P<0.05或0.01)。结论 重大公共卫生事件对住院青少年精神障碍疾病构成比率产生了影响,青少年情感性精神障碍患者增加明显,但其并不是住院青少年精神障碍患者构成比率变化的独立危险因素。
Objective To analyze the changes in kinds of adolescent mental disorders hospitalized in a class A of tertiary psychiatric hospital last 3 years in order to provide reference for the prevention of adolescent mental d iseases.Methods Adolescent patients hospitalized in a class A of tertiary psychiatric hospital last 3 years were set as study group and ones in the previous 3 years as control group,homepage data of medical records were analyzed,and patients' general information and changes in illness composition summarized and contrasted.Results A total of 8 889 patients were included,3 584 cases in control and 5 305 in study group,illness kinds of the two groups were roughly the same,difference in the cis-position of illness kinds wasn't great,but the proportion of non-suicidal self-injury as well as the number of first visits and hospitalization expenditure were higher in study than control group(P<0.01).The proportion of depressive episode in kind changes of affective disorders was higher in study than control group(P<0.01).Multiple Logistic regression analysis showed that genders,child abuse,types of hou sehold registration,family history,a nd major public health events(MPHE) were factors influencing the changes in the proportion of mental d isorder s to d if ferent deg ree(P<0.05 or 0.01).Conclusion The M PH E has an influence on constituent ratio of hospitalized adolescent mental disorders,adolescents with affective disorders significantly increase,however it isn't an independent risk factor for the change in the composition of hospitalized adolescent mental disorders.
