摘要
目的 观察麦粒灸联合地榆升白片治疗肺癌放疗后白细胞减少症的临床疗效。方法 将92例肺癌放疗后白细胞减少症患者,随机分为观察组和对照组,每组46例。在常规对症治疗的基础上,对照组给予地榆升白片治疗,观察组在对照组基础上给予麦粒灸治疗。观察两组治疗前后中医证候积分、白细胞计数和中性粒细胞计数、T细胞亚群水平的变化,并比较两组临床疗效及骨髓抑制程度。结果 观察组总有效率为91.3%,显著高于对照组的78.3%(P<0.05)。两组治疗后中医证候积分较治疗前降低(P<0.05),且观察组低于对照组(P<0.05)。两组治疗后血白细胞计数和中性粒细胞计数均较治疗前升高(P<0.05),且观察组高于对照组(P<0.05)。两组治疗后CD3^(+)T细胞百分比、CD4^(+)T细胞百分比、CD4^(+)/CD8^(+)比值显著降低(P<0.05),CD8^(+)T细胞百分比升高(P<0.05),且观察组优于对照组(P<0.05)。观察组骨髓抑制总发生率为34.8%,低于对照组的60.9%(P<0.05)。结论 在常规对症治疗的基础上,麦粒灸联合地榆升白片可改善肺癌放疗后白细胞减少症,临床疗效显著。
Objective To observe the clinical efficacy of wheat-grain size cone moxibustion plus Di Yu Sheng Bai tablet for leukopenia after radiotherapy for lung cancer.Method Ninety-two leukopenia patients were randomized to observation and control groups,with 46 cases in each group.In addition to routine symptomatic treatment,the control group was treated with Di Yu Sheng Bai tablet and the observation group,by wheat-grain size cone moxibustion in addition.The TCM syndrome score,white blood cell count,neutrophil count and T cell subset levels were observed in the two groups before and after treatment.The clinical therapeutic effects and the degrees of myelosuppression were compared between the two groups.Result The total efficacy rate was 91.3%in the observation group,which was significantly higher than78.3%in the control group(P<0.05).After treatment,the TCM syndrome score decreased in the two groups compared with before(P<0.05)and was lower in the observation group than in the control group(P<0.05).After treatment,white blood cell count and neutrophil count increased in the two groups compared with before(P<0.05)and were higher in the observation group than in the control group(P<0.05).After treatment,the percentages of CD3^(+)T cells and CD4^(+)T cells and the ratio of CD4^(+)/CD8^(+)decreased significantly and the percentage of CD8^(+)cells increased in the two groups(P<0.05);they were better in the observation group than in the control group(P<0.05).The total incidence of bone marrow transplantation was 34.8%in the observation group,which was lower than 60.9%in the control group(P<0.05).Conclusion In addition to routine symptomatic treatment,wheat-grain size cone moxibustion plus Di Yu Sheng Bai tablet can relieve leukopenia after radiotherapy for lung cancer with a marked therapeutic effect clinically.
