摘要
目的:观察通腑宣肺汤加味治疗儿童肺炎支原体肺炎痰热闭肺证的临床效果。方法:选择80例肺炎支原体肺炎痰热闭肺证患儿为研究对象,按随机数表法分为观察组和对照组各40例。2组均采用常规对症处理措施,对照组在常规治疗基础上采用阿奇霉素治疗,观察组在对照组基础上给予通腑宣肺汤加味治疗,2组均治疗7 d。评价2组临床疗效,比较2组症状改善时间,比较2组治疗前后中医证候积分、血清降钙素原(PCT)、C-反应蛋白(CRP)、白细胞介素-6 (IL-6)的水平,观察不良反应发生情况。结果:观察组总有效率为92.50%,高于对照组75.00%(P<0.05)。观察组咳嗽消失时间及肺啰音消失时间均短于对照组(P<0.05)。治疗后,2组主症、次症评分及中医证候总积分较治疗前降低(P<0.05),且观察组主症、次症积分及中医证候总积分均低于对照组(P<0.05)。治疗后,2组血清PCT、CRP、IL-6水平较治疗前降低(P<0.05),且观察组血清PCT、CRP、IL-6水平均低于对照组(P<0.05)。2组治疗期间均无严重不良反应发生。结论:通腑宣肺汤加味治疗儿童肺炎支原体肺炎痰热闭肺证的临床效果明显,能有效缓解症状,降低血清炎症因子水平,安全可靠。
Objective:To observe the clinical effect of modified Tongfu Xuanfei Decoction on mycoplasma pneumoniae pneumonia with syndrome of phlegm-heat obstructing the lung in children.Methods:A total of 80 children with mycoplasma pneumoniae pneumonia with syndrome of phlegm-heat obstructing the lung were selected as the study subjects,and divided into the observation group and the control group according to the random number table method,with 40 cases in each group.The two groups were given routine symptomatic treatment;the control group was additionally treated with Azithromycin based on routine treatment,and the observation group was additionally treated with modified Tongfu Xuanfei Decoction based on the treatment of the control group.The two groups were treated for 7 days.The clinical effects in both groups were evaluated;the improvement time of symptoms,traditional Chinese medicine(TCM) syndrome scores,and levels of serum procalcitonin(PCT),C-reactive protein(CRP) and interleukin-6(IL-6) were compared before and after treatment between the two groups;the incidence of adverse reactions was also observed.Results:The total effective rate was 92.50% in the observation group,higher than that of 75.00% in the control group(P<0.05).The disappearance time of cough and pulmonary rales was respectively shorter than that in the control group(P<0.05).After treatment,the scores of primary and secondary symptoms and total scores of TCM syndromes in the two groups were decreased when compared with those before treatment(P<0.05),and the above scores in the observation group were lower than those in the control group(P<0.05).After treatment,the levels of serum PCT,CRP and IL-6 in the two groups were decreased when compared with those before treatment(P<0.05),and the levels in the observation group were lower than those in the control group(P<0.05).There was no serious adverse reaction during treatment in the two groups.Conclusion:Modified Tongfu Xuanfei Decoction has a significant clinical effect on mycoplasma pneumoniae pneumonia with syndrome of phlegm-heat obstructing the lung in children,which can effectively mitigate the symptoms,and decrease the levels of serum inflammatory factors with good safety.
