摘要
目的 探讨复合益生菌对单侧全膝关节置换术后患者的影响。方法 选取2021年1—12月于河北医科大学第三医院行单侧全膝关节置换手术患者92例,采用随机、双盲、安慰剂对照的方法分为2组,随访过程中对照组脱落2例,试验组脱落1例,最终对照组44例、试验组45例完成研究。术后第1天开始对照组患者服用麦芽糊精,2次/d,3 g/次,连续服用14 d;试验组患者服用复合益生菌,2次/d,3 g/次,连续服用14 d。观察2组患者术后不同时间点胃肠道症状分级评分量表(GSRS)评分、疼痛VAS评分、关节炎严重程度WOMAC评分、生活质量SF-36评分以及血液相关指标的变化情况。结果 2组患者组内不同时间GSRS评分比较差异均有统计学意义(P均<0.05),试验组患者组内术后第3天GSRS评分明显低于对照组(P<0.05)。2组患者组内不同时间VAS评分比较差异均有统计学意义(P均<0.05),试验组患者术后第14天VAS评分明显低于对照组(P<0.05)。2组患者组内不同时间WOMAC评分比较差异均有统计学意义(P均<0.05),2组患者术后各时间点WOMAC评分比较差异均无统计学意义(P均>0.05)。2组患者术后不同时间点SF-36总分及生理功能评分、活力评分、社会功能评分、心理健康评分比较差异均有统计学意义(P均<0.05),2组患者术后不同时间点生理职能评分、躯体疼痛评分、总体健康评分以及情感职能评分比较差异均无统计学意义(P均>0.05);试验组患者术后第14天SF-36总分及活力评分、心理健康评分均明显高于对照组(P均<0.05)。2组患者组内术后不同时间点白细胞计数、单核细胞百分比、中性粒细胞/淋巴细胞比值以及C反应蛋白水平比较差异均有统计学意义(P均<0.05),2组患者术后第7天白细胞计数、单核细胞百分比、中性粒细胞/淋巴细胞比值以及C反应蛋白水平比较差异均无统计学意义(P均>0.05)。结论 服用复合益生菌可改善单侧全膝关节置换术后患者的胃肠道症状以及疼痛情况,提高患者的生活质量及心理健康状况。
Objective It is to investigate the effect of compound probiotics in patients after unilateral total knee arthroplasty.Methods Ninety-two patients who underwent unilateral total knee arthroplasty in the Third Hospital of Hebei Medical University from January to December 2021 were selected and divided into 2 groups using a randomized,double-blind,placebo-controlled method,with 2 cases lost from the control group and 1 case lost from the experimental group during following-up,and finally 44 cases in the control group and 45 cases in the experimental group completed the study.Starting from the 1st postoperative day,the patients in the control group were given maltodextrin and those in the experimental group were given compound probiotics,both twice per day,3 g per time,continuously treated for 14 days.The gastrointestinal symptom rating scale(GSRS) scores,pain VAS scores,WOMAC scores,SF-36 scores,and blood-related indicators of the patients in the two groups were observed at different postoperative time points.Results The differences in GSRS score,VAS scores WOMAC scores at different times were statistically significant in each group(all P<0.05),and the GSRS scores at postoperative day 3,the VAS scores at postoperative day 14 in the experimental group were significantly lower than those in the control group(P<0.05),but there were no significant differences in the WOMAC scores at different postoperative time points between the two groups(all P>0.05).The differences in the total scores and physiological function scores,vitality scores,social function scores,and mental health scores of SF-36 at different postoperative time points were statistically significant between the two groups(all P<0.05),and the differences in the physical function scores,somatic pain scores,general health scores,and emotional function scores at different postoperative time points were not statistically significant between the two groups(all P>0.05);the total scores,vitality scores,and mental health scores of SF-36 of patients in the experimental group were significantly higher than those in the control group at postoperative day 14(all P<0.05).The differences in leukocyte counts,monocyte percentages,neutrophil/lymphocyte ratios,and the level of C-reactive protein at different postoperative time points were statistically significant in each group(all P<0.05),and the differences in leukocyte counts,monocyte percentages,neutrophil/lymphocyte ratios,and the level of C-reactive protein at the postoperative day 14 were not statistically significant between the two groups(all P>0.05).Conclusion Compound probiotics can improve the gastrointestinal symptoms,pain,quality of life and psychological health of patients after unilateral total knee arthroplasty.
