双黄连氧气驱动雾化吸入联合NCPAP治疗对MP合并呼吸衰竭患儿的影响

The Effect of Shuanghuanglian Oxygen Driven Nebulization Inhalation Combined with Nasal Continuous Positive Airway Pressure in the Treatment of Mycoplasma Pneumonia Complicated with Respiratory Failure in Children

摘要 目的探讨采取双黄连氧气驱动雾化吸入联合经鼻持续气道正压通气(NCPAP)治疗儿童重症支原体肺炎(MP)合并呼吸衰竭的效果及对血清瘦蛋白(Leptin)、白细胞介素⁃17(IL⁃17)及白细胞介素⁃33(IL⁃33)水平的影响。方法选取82例患儿并随机分成观察组42例与对照组40例,对照组采取常规吸氧、止咳及NCPAP治疗,观察组则是在对照组的基础上加用双黄连氧气驱动雾化吸入治疗,比较2组治疗效果。结果观察组患儿各症状消退时间明显比对照组短(P<0.05);治疗后2组血氧饱和度(SaO_(2))、血氧分压(PaO_(2))较治疗前提高,二氧化碳分压(PaCO_(2))较治疗前降低,而观察组指标变化幅度较对照组显著(P<0.05);治疗后2组血清Leptin、IL⁃17、IL⁃33均较治疗前降低,观察组各指标显著低于对照组(P<0.05)。结论双黄连氧气驱动雾化吸入联合NCPAP治疗儿童重症MP合并呼吸衰竭,可显著改善患儿血气功能指标,降低血清炎症因子水平,促进患儿症状的改善。 Objective To investigate the effect of using Shuanghuanglian oxygen⁃driven nebulized inhalation combined with nasal continuous positive airway pressure(NCPAP)in the treatment of severe mycoplasma pneumonia(MP)complicated with respiratory failure in children and its influence on the levels of Leptin,interleukin⁃17(IL⁃17)and interleukin⁃33(IL⁃33).Methods Eighty⁃two children were selected and randomly divided into observation group of 42 cases and control group of 40 cases.The control group was treated with conventional oxygen inhalation,cough relieving and NCPAP treatment,and the observation group was treated with Shuanghuanglian oxygen driven nebulization on the basis of the control group.The effect of 7d treatment between the two groups was compared.Results The time of symptom resolution in observation group was shorter than that in control group(P<0.05).After treatment,the blood oxygen saturation(SaO_(2))and blood oxygen partial pressure(PaO_(2))of the 2 groups were higher than before treatment,and the partial pressure of carbon dioxide(PaCO_(2))was lower than before treatment,and the indexes of the observation group were significantly changed compared with the control group(P<0.05).After treatment,serum Leptin,IL⁃17 and IL⁃33 in the 2 groups were lower than those before treatment,and the indexes in the observation group were significantly lower than those in the control group(P<0.05).Conclusion Shuanghuanglian oxygen⁃driven nebulized inhalation combined with NCPAP in the treatment of severe MP complicated with respiratory failure in children can significantly improve the blood gas function of children,reduce the level of serum inflammatory factors,and promote the improvement of children’s symptoms.
