摘要
目的运用中医传承辅助平台(V2.5)对庞敏教授治疗心肌桥的医案进行数据挖掘,总结其用药组方规律和学术思想。方法收集庞敏教授治疗心肌桥的有效病案,运用中医传承辅助平台进行用药规律分析,对核心药物组合进行网络药理学分析其治疗心肌桥的作用机制。结果心肌桥的发作高峰为9月至次年4月,最高为11月,次之为2月;发作曲线与沈阳地区平均气温呈负相关。共筛选98首处方,使用频次最高中药为柴胡、牛膝、桔梗、枳壳。中药功效主要为理气药,其次为活血化瘀药和补虚药。中药四气以温为主,其次为寒、平;五味以苦、辛、甘味为主;归经以肝经为主,其次为心经、脾经。核心药物组合为柴胡-桔梗-枳壳-牛膝。通过聚类得到新处方10个。结论庞敏教授认为气机失调为心肌桥发作的根本诱因,以“调畅气机法”为基本治则,核心药物组合为柴胡-桔梗-枳壳-牛膝,同时运用辨天、辨人、辨证、辨机、辨病的“五位一体”理论体系提供辨证诊疗思路。
Objective To use the TCM Inheritance Support System(V2.5)to data-mine professor PANG Min′s medical cases for the treatment of myocardial bridges and summarize his prescription patterns and academic ideas.Methods The effective cases of myocardial bridges treated by professor PANG Min were collected,and the pattern of drug use was analyzed by using the TCM Inheritance Support System,and the mechanism of action of core drug combinations was analyzed by network pharmacology.Results The peak of myocardial bridge episodes was from September to April of the next year,with the highest in November and the second highest in February;the episodes curve was negatively correlated with the average temperature in Shenyang area.A total of 98 prescriptions were screened,and the most frequently used herbs were Chaihu(Bupleuri Radix),Niuxi(Achyranthis Bidentatae Radix),Jiegeng(Platycodonis Radix)and Zhiqiao(Aurantii Fructus).The efficacy of Chinese herbal medicines was mainly for regulating Qi,followed by activating blood circulation and removing blood stasis and tonifying deficiency.The four natures of Chinese herbs are mainly warm,followed by cold and flat;the five tastes are mainly bitter,pungent and sweet;and the normalizing meridians are mainly liver meridians,followed by heart and spleen meridians.The core drug combination was Chaihu(Bupleuri Radix)-Jiegeng(Platycodonis Radix)-Zhiqiao(Aurantii Fructus)-Niuxi(Achyranthis Bidentatae Radix).Ten new prescriptions were obtained by clustering.Conclusion Professor PANG Min believed that Qi imbalance is the root cause of myocardial bridge attack,and“regulating Qi-flowing method”is the basic rule of treatment.The core drug combination was Chaihu(Bupleuri Radix)-Jiegeng(Platycodonis Radix)-Zhiqiao(Aurantii Fructus)-Niuxi(Achyranthis Bidentatae Radix).The“five in one”theoretical system of differentiation of heaven,man,syndrome and disease is used to provide diagnosis and treatment ideas.
