中医药调控缺血缺氧微环境对青光眼治疗及视网膜神经节细胞凋亡的研究进展

摘要 青光眼是首位不可逆性致盲眼病,严重威胁人类视觉健康。当前常规治疗方法是使用眼药水、手术等方式以降低眼内压,但是临床长期疗效差强人意。青光眼最主要的病理基础为神经节细胞(retinal ganglion cells,RGCs)退行性改变及凋亡,临床及实验研究发现缺血缺氧微环境的改变常常会加重RGCs的凋亡,而如何延缓甚至抑制其凋亡是当前研究的热点。气血理论是中医核心概念之一,中医药具有多组分、多途径、多靶标的作用特点,对于复杂病理机制所导致的视网膜缺血缺氧微环境改变具有独特的治疗优势和潜力。随着近年来中医药研究的不断发展,中药在保护RGCs方面取得了不少新的研究。该文就中药有效活性成分和复方制剂在保护RGCs的有效性及潜在机制方面进行了总结,以期为临床工作中更有效地抑制RGCs凋亡、改善视功能提供一定的借鉴意义。 Glaucoma is the first irreversible cause of blindness,which is a serious threat to human visual health.The conventional treatment is the use of eye drops,surgery and other methods to reduce intraocular pressure at present,but the long-term clinical effect is not satisfactory.The main pathological basis of glaucoma is degeneration and apoptosis of retinal ganglion cells(RGCs).Clinical and experimental studies have found that the changes of ischemia-hypoxia microenvironment often aggravate the apoptosis of RGCs,and how to delay or even inhibit the apoptosis of RGCs is the current research hotspot.Traditional Chinese Medicine(TCM)has the characteristics of multi-components,multi-pathways and multi-targets,and has unique therapeutic advantages and potential for the changes of retinal ischemia and hypoxia microenvironment caused by complicated pathological mechanism.With the continuous development of TCM research in recent years,TCM has made a lot of new research in the protection of RGCs.This paper summarized the effectiveness and potential mechanism of TCM active components and compound preparations in protecting RGCs,in order to provide some reference for clinical work to more effectively inhibit RGCs apoptosis and improve visual function.
