摘要
目的探究运动发育迟缓儿童血碱性磷酸酶(ALP)、血25-羟维生素D3[25(OH)D3]表达及其与大肌肉群动作发展水平的相关性。方法回顾性选取2020年1月至2022年2月在邯郸市妇幼保健院接受治疗的78例运动发育迟缓儿童作为研究组,另选78名发育正常的健康儿童纳入对照组。比较两组儿童的入院时血ALP、25(OH)D3表达及大肌肉群动作发展水平。对研究组儿童进行常规治疗和随访,记录患儿治疗前后的血ALP、25(OH)D3表达、Gesell发育量表评分及大肌肉群动作发展水平变化,并探究其相关性。结果研究组儿童的血ALP和25(OH)D3水平分别为(177.26±12.98)U/L、(25.11±3.19)nmol/L,均低于对照组[(233.21±23.16)U/L、(32.05±2.46)nmol/L],差异均有统计学意义(P<0.05)。研究组儿童在大动作、个人社交、精细运动、语言、适应性行为等方面评分分别为(74.35±14.26)、(66.29±13.54)、(62.59±11.48)、(52.99±12.64)、(55.42±13.28)分,均低于对照组[(96.55±13.27)、(88.87±12.05)、(99.26±12.33)、(88.26±13.29)、(96.66±14.26)分],差异均有统计学意义(P<0.05)。研究组儿童的移动性、控制性及总分分别为(22.15±2.28)、(21.06±2.59)、(45.56±4.59)分,均低于对照组[(33.26±4.26)、(30.66±4.11)、(70.26±11.26)分],差异均有统计学意义(P<0.05)。经Pearson相关性分析,TGMD-3总分与其血ALP和25(OH)D3水平呈正相关(r=0.832、0.775,P<0.05)。与治疗前相比,研究组儿童治疗后的血清ALP和25(OH)D3水平均显著升高,大动作、个人社交、精细运动、语言、适应性行为等方面的评分,移动性、控制性及总分方面均显著升高,差异均有统计学意义(P<0.05)。结论运动发育迟缓的儿童血清ALP和25(OH)D3水平较发育正常儿童明显降低,且与其大肌肉群动作发展水平呈正相关;在治疗过程中,提高ALP和25(OH)D3水平可能对促进儿童的大肌肉群动作发展有积极的影响,应密切关注。
Objective To explore the expression of blood alkaline phosphatase(ALP),blood 25-hydroxyvitamin D3[25(OH)D3]in children with motor development delay and their correlation with the level of motor development of large muscle groups.Methods A total of 78 children with motor retarding who received treatment in Handan Maternal and Child Health Hospital from January 2020 to February 2022 were selected as the study group,and 78 healthy children with normal development were selected as the control group.The children in the study group received routine treatment,and the expression levels of blood ALP,25(OH)D3 and motor development of large muscle groups were compared between the two groups.The patients in the study group were followed up,and the changes of blood ALP,25(OH)D3 expression,Gesell developmental scale score and motor development level of large muscle groups before and after treatment were recorded,and the correlation was explored.Results The levels of serum ALP and 25(OH) D3 of children in the study group were(177.26±12.98) U/L and(25.11±3.19) nmol/L,respectively,which were lower than those in the control group [(233.21±23.16) U/L and(32.05±2.46) nmol/L],and the differences were statistically significant(P<0.05).The scores of gross motor,personal social,fine motor,language,and adaptive behavior of children in the study group were(74.35±14.26),(66.29±13.54),(62.59±11.48),(52.99±12.64),and(55.42±13.28) points,respectively,which were lower than those in the control group [(96.55±13.27),(88.87±12.05),(99.26±12.33),(88.26±13.29),and(96.66±14.26) points],and the differences were statistically significant(P<0.05).The mobility,control,and total scores of children in the study group were(22.15±2.28),(21.06±2.59),and(45.56±4.59) points,respectively,which were lower than those in the control group[(33.26±4.26),(30.66±4.11),and(70.26±11.26) points],and the differences were statistically significant(P<0.05).By Pearson correlation analysis,the total score of TGMD-3 was positively correlated with the serum ALP and 25(OH)D3 levels(r=-0.832,-0.775,P<0.05).The levels of serum ALP and 25(OH)D3 of children in the study group after treatment were higher than those before treatment,the scores of gross motor,personal social,fine motor,language and adaptive behavior,and mobility,control and total score were higher than those before treatment,the differences were statistically significant(P<0.05).Conclusion Serum ALP and 25(OH)D3 levels of children with motor retarding are significantly lower than those in children with normal motor development,and there is a positive correlation with the motor development level of their gross muscle groups.During treatment,increasing blood ALP and 25(OH)D3 levels may have a positive effect on promoting the motor development of children's gross muscle groups,which should be paid close attention to.
