摘要
目的观察麝香保心丸联合稳心颗粒治疗急性心肌梗死(AMI)致心律失常的临床疗效。方法选取2022年10月—2023年10月湘南学院附属医院收治的AMI致心律失常患者120例,按照随机抽签法分为单一治疗组与联合治疗组,各60例。单一治疗组予以稳心颗粒,联合治疗组予以麝香保心丸联合稳心颗粒,2组均持续用药4周。比较2组临床疗效,用药前、用药4周后心率变异性指标[相邻正常RR间期差值>50 ms的百分比(PNN50)、相邻正常RR间期差值的均方根(RMSSD)、24 h内RR间期的标准差(SDNN)和24 h内每5 min RR间期平均值标准差(SDANN)]、异位搏动情况(室性异位搏动和室上性异位搏动),不良反应。结果联合治疗组治疗总有效率高于单一治疗组(95.00%vs.80.00%,χ^(2)=6.171,P=0.013)。用药4周后,2组PNN50较用药前升高,RMSSD、SDNN、SDANN较用药前增加,且联合治疗组升高/增加幅度大于单一治疗组(P<0.05或P<0.01);2组室性异位搏动、室上性异位搏动次数较用药前减少,且联合治疗组少于单一治疗组(P<0.01)。联合治疗组与单一治疗组不良反应总发生率比较,差异无统计学意义(11.67%vs.8.33%,χ^(2)=0.370,P=0.543)。结论AMI致心律失常患者采用麝香保心丸联合稳心颗粒治疗可提高疗效,改善心率变异性,减轻室性异位搏动,且安全性较高。
Objective To observe clinical effect of Shexiang Baoxin pills combined with Wenxin granules for arrhythmia caused by AMI.Methods A total of 120 cases of patients with arrhythmia caused by AMI were selected from October 2022 to October 2023 in Affiliated Hospital of Xiangnan University,which were randomly divided into monotherapy group and combined treatment group,with 60 cases in each group.The monotherapy group was given Wenxin granules,and the combined treatment group was given Shexiang Baoxin pills combined with Wenxin granules.Both groups continued to be treated for 4 weeks.The clinical efficacy,heart rate variability(PNN50,RMSSD,SDNN and SDANN),ectopic beats(ventricular ectopic beats and supraventricular ectopic beats)before and after medication of 4 weeks and adverse reactions were compared between the two groups.Results The total effective rate in the combined treatment group was higher than that in the monotherapy group(95.00%vs.80.00%,χ2=6.171,P=0.013).After 4 weeks of medication,the PNN50 in the two groups was higher than that before treatment,RMSSD,SDNN and SDANN were more than those before treatment,and the increase/extension range of the combined treatment group were greater than those of the monotherapy group(P<0.05 or P<0.01).The ventricular ectopic beats and supraventricular ectopic beats in the two groups were less than those before treatment,and those in the combined treatment group were lower than the monotherapy group(P<0.01).There was no significant difference in the total incidence of adverse reactions between the combined treatment group and the monotherapy group(11.67%vs.8.33%,χ2=0.370,P=0.543).Conclusion Shexiang Baoxin pills combined with Wenxin granules can improve the curative effect for arrhythmia caused by AMI,it can improve heart rate variability,reduce ventricular ectopic beats,and with high safety.
