摘要
目的分析输血次数与多次输血血液病患者输注效果的关系。方法选取2020年5月—2023年7月菏泽市立医院收治的600例多次输血(≥2次)的血液病患者作为研究对象,根据输血次数不同将患者分为A组225例(2~3次)、B组195例(4~6次)及C组180例(≥7次)。比较3组患者血小板抗体阳性率;比较血小板抗体阳性与阴性患者输注血小板后1、24 h校正血小板计数增加值(CCI)及输注有效率。结果600例患者中共226例患者检测血小板抗体阳性,阳性率为37.67%,其中C组血小板抗体阳性率高于A、B组(P<0.01)。血小板抗体阳性者输注后1、24 h CCI及输注有效率均低于血小板阴性患者(P<0.01)。结论输血次数越多,血液病患者血小板抗体阳性率越高,而血小板抗体阳性患者的血小板输注效果较差,而血小板抗体筛查能有效提高输血效果。
Objective To analyze the relationship of blood transfusion times and the transfusion effect of patients with multiple blood transfusions.Methods A total of 600 blood disease patients who required multiple blood transfusions(≥2 times)were admitted to Heze Municipal Hospital from May 2020 to July 2023 were selected as the research subjects,and they were divided into group A with 225 cases(2-3 times),group B with 195 cases(4-6 times)and group C with 180 cases(≥7 times)based on the times of blood transfusion.The positive rate of platelet antibody were compared among the three groups,and CCI at 1,24 h after platelet transfusion,infusion efficiency were compared between the patients with positive and negative of platelet antibody.Results Among 600 cases,226 were detected positive platelet antibody,with rate of 37.67%,and the positive rate of platelet antibody of group C was higher than groups A,B(P<0.01).CCI at 1,24 h after platelet transfusion,infusion efficiency rate of patients with positive platelet antibody were lower than those of patients with negative of platelet antibody(P<0.01).Conclusion The more transfusion times,the higher the positive rate of platelet antibody in patients with blood disease,while the platelet transfusion effect of patients with positive platelet antibody is poor,and platelet antibody screening can effectively improve the blood transfusion effect.
