摘要
目的探讨放射性碘联合丹参注射液+小剂量甲巯咪唑治疗甲亢性心脏病的有效性和安全性。方法选取医院心内科2022年1月至2023年3月收治的甲亢性心脏病患者120例,随机分为试验组和对照组,各60例。对照组患者在对症治疗基础上予^(131)I治疗,试验组患者在对照组基础上联用丹参注射液+小剂量甲巯咪唑。对比两组患者的甲状腺功能[促甲状腺激素受体抗体(TRAb)、促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)与血清游离甲状腺素(FT4)]和心功能指标[左室射血分数(LVEF)、左心室短轴缩短率(LVFS)及心功能分级],以及不良反应发生情况。结果两组患者治疗前的FT3和FT4组间无显著差异,且均较正常水平高;治疗后,试验组患者的FT3及FT4指标恢复正常较对照组更快。治疗后,试验组患者的TSH和TRAb恢复均较对照组更快,LVEF和LVFS的改善均较对照组明显,心功能评价指标好转较对照组更快(P<0.05)。结论放射性碘联合丹参注射液+小剂量甲巯咪唑治疗甲亢性心脏病能明显缩短治疗时间,提升治疗有效率,且安全性较高。
Objective To investigate the efficacy and safety of radioactive iodine combined with Danshen Injection and low-dose methimazole in the treatment of hyperthyroidism heart disease.Methods A total of 120 patients with hyperthyroidism heart disease admitted to the cardiology department of the hospital from January 2022 to March 2023 were selected and randomly divided into the experimental group and the control group,with 60 cases in each group.On the basis of symptomatic treatment,the control group was treated with ^(131)I,while the experimental group patients were treated with Danshen Injection combined with low-dose Methimazole on the basis of the control group.The thyroid function(thyroid stimulating hormone receptor antibody(TRAb),thyroid stimulating hormone labeling(left) ventricular ejection fraction(LVEF),left ventricular short axis shortening rate(LVFS),cardiac function grading)of patients,and the incidence of adverse reactions between two groups were compared.Results There was no significant difference in FT3 and FT4 levels between the two groups before treatment,and both were higher than normal levels.After treatment,the recovery of FT3 and FT4 indicators of patients in the experimental group was faster than that in the control group.After treatment,the recovery of TSH and TRAb in the experimental group was faster than that in the control group,and the improvement of LVEF and LVFS was more significant than that in the control group.The improvement of cardiac function evaluation indicators was faster than that in the control group(P<0.05).Conclusion Radioactive iodine combined with Danshen Injection and low-dose methimazole can significantly shorten the treatment time,improve the treatment efficiency,and have high safety in the treatment of hyperthyroidism heart disease.
