摘要
目的 探讨促性腺释放激素激动剂联合补肾活血散瘀汤治疗子宫腺肌病的临床效果。方法选取2018—2022年南京中医药大学附属江苏省中西医结合医院、溧阳市中医医院、泰州市中医院、南通市中医院、苏州市中西医结合医院、镇江市中西医结合医院6所医院的44例肾虚血瘀型子宫腺肌病患者,按照随机信封法分为三组。A组予以促性腺释放激素激动剂(15例),B组予以补肾活血散瘀汤(21例),C组予以促性腺释放激素激动剂联合补肾活血散瘀汤(8例)。排除脱落病例,最终纳入研究病例为A组14例,B组19例,C组7例,比较各组子宫体积、痛经积分、月经量积分及肾虚腰痛积分。结果 治疗后,三组子宫体积较治疗前缩小(P <0.05)。治疗后,三组子宫体积比较,差异无统计学意义(P> 0.05)。治疗后,C组痛经积分低于A组和B组(P <0.05)。治疗后,C组月经量积分低于B组(P <0.05)。治疗后,B组肾虚腰痛积分低于A组(P<0.05)。三组均未出现临床检验指标(血常规、生化、尿常规)异常及相关药物不良反应。结论 促性腺释放激素激动剂联合补肾活血散瘀汤治疗子宫腺肌病,可缩小子宫体积,改善痛经、肾虚腰痛症状,减少月经量,且具有安全性,可推广应用。
Objective To investigate the clinical effect of gonadotropin releasing hormone agonist combined with Bushen Huoxue Sanyu Decoction in the treatment of adenomyosis.Methods A total of 44 patients with kidney deficiency and blood stasis type adenomyosis from 6 hospitals of Jiangsu Province Hospital of Integrative Chinese and Western Medicine Affiliated to Nanjing University of Chinese Medicine,Liyang Hospital of Chinese Medicine,Taizhou Hospital of Traditional Chinese Medicine,Nantong Hospital of Traditional Chinese Medicine,Suzhou Hospital of Integrated Traditional Chinese and Western Medicine and Zhenjiang Hospital of Integrated Traditional Chinese and Western Medicine from 2018 to 2022 were selected and divided into three groups according to random envelope method.Group A was given gonadotropin releasing hormone agonist(15 cases),group B was given Bushen Huoxue Sanyu Decoction(21 cases),group C was given gonadotropin releasing hormone agonist combined with Bushen Huoxue Sanyu Decoction(8 cases).Excluding shedding cases,the final included cases were 14 cases in group A,19 cases in group B and 7 cases in group C.The uterine volume,dysmenorrhea score,menstrual volume score and kidney deficiency lumbago score of all groups were compared.Results After treatment,the uterine volume of the three groups was smaller than that before treatment(P<0.05).After treatment,there was no significant difference in uterine volume among the three groups(P>0.05).After treatment,dysmenorrhea score in group C was lower than that in groups A and B(P<0.05).After treatment,the score of menstrual volume in group C was lower than that in group B(P<0.05).After treatment,the score of kidney deficiency lumbago in group B was lower than that in group A(P<0.05).No abnormal clinical test indexes(blood routine,biochemistry,urine routine)and related adverse drug reactions were found in the three groups.Conclusion Gonadotropin releasing hormone agonist combined with Bushen Huoxue Sanyu Decoction in the treatment of adenomyosis can reduce uterine volume,improve dysmenorrhea,kidney deficiency lumbago symptoms,reduce menstrual volume,and have safety,whi ch can be widely used.
