摘要
目的:基于IVIM逐层匀场(iShim-IVIM)和常规DWI技术,对比分析各定量参数鉴别甲状腺良、恶性结节的价值。方法:前瞻性收集58例拟行手术切除的甲状腺结节的MRI资料。比较iShim-IVIM和DWI参数:纯扩散系数(D)、伪扩散系数(D^(*))、灌注分数(f)与表观扩散系数(ADC)值在甲状腺良恶性结节中的差异。对差异显著者绘制受试者工作特征(ROC)曲线,比较各参数的诊断效能。结果:共58个甲状腺结节中良恶性分别占29个,两组间ADC、D、f值差异显著(P<0.05),D^(*)值无明显差异(P>0.05);D值鉴别甲状腺良恶性结节的诊断效能最高,AUC为0.883。ADC与D之间存在正的强相关(r=0.752,P<0.001),ADC与f、D^(*)之间无显著相关性(r=0.231、0.092,P=0.081、0.492)。结论:基于体素内不相干运动逐层匀场(iShim-IVIM)技术为甲状腺良、恶性结节的鉴别诊断提供更多信息,D与常规DWI生成的ADC值相比具有更高价值。
Objective:Based on iShim-IVIM and conventional DWI technology,to compare the value of each quantitative parameter in identifying benign and malignant thyroid nodules.Methods:MRI data of 58 cases of thyroid nodules to be surgically resected were prospectively collected.Compare iShim-IVIM and DWI parameters:Pure diffusion coefficient(D),pseudo-diffusion coefficient(D^(*)),perfusion fraction(f)and apparent diffusion coefficient(ADC)values in benign and malignant thyroid nodules.Receiver operating characteristic(ROC)curve was drawn for those with statistically significant differences,and the diagnostic efficiency of each parameter was compared.Results:There were 58 nodules,including 29 benign nodules and 29 malignant nodules.The differences in ADC,D,and f values between the two groups were statistically significant(P<0.05),while the difference in D*value was not statistically significant(P>0.05).The D value had the highest diagnostic efficiency for distinguishing benign and malignant thyroid nodules,with an AUC of 0.883.There was a positive strong correlation between ADC and D(r=0.752,P<0.001),and no significant correlation between ADC and f,D^(*)(r=0.231,0.092,P=0.081,0.492).Conclusion:Based on voxel-incoherent motion integrated slice-by-slice shimming(iShim-IVIM)technology,more information is provided for the differential diagnosis of benign and malignant thyroid nodules,and D has greater value than ADC generated by conventional DWI.
