摘要
目的分析颈动脉血管超声对冠心病(CHD)患者冠状动脉狭窄程度的评估价值。方法回顾性分析2021年1月—2023年10月医院收治的60例确诊为CHD的患者临床资料,所有患者均行颈动脉血管超声检查和冠状动脉造影(CAG)检查,以CAG为金标准,根据冠状动脉狭窄程度将患者分为冠状动脉狭窄程度≥75%组(n=45)与冠状动脉狭窄程度<75%组(n=15)。比较不同冠状动脉狭窄程度的CHD患者颈动脉双侧距离分叉处1 cm颈总动脉内中膜厚度(IMTc)、颈动脉分叉处内中膜厚度(IMTs)、斑块形成情况,并采用受试者工作特征曲线(ROC)分析颈动脉血管超声参数对CHD患者冠状动脉狭窄程度的评估效能。结果冠状动脉狭窄程度<75%的CHD患者IMTc、IMTs、斑块形成均少于冠状动脉狭窄程度≥75%患者(P<0.05);IMTc、IMTs、斑块形成评估CHD患者冠状动脉狭窄程度时,以斑块形成的AUC最高,为0.889,并显著高于IMTc、IMTs,其评估CHD患者冠状动脉狭窄程度的敏感度、特异度分别为84.44%、93.33%。结论颈动脉血管超声参数IMTc、IMTs、斑块形成情况评估CHD患者冠状动脉狭窄程度具有一定实用性,其中斑块形成的高特异度值得临床重视。
Objective To analyze the value of carotid artery ultrasound in evaluating the degree of coronary artery stenosis in patients with coronary heart disease(CHD).Methods A retrospective analysis was conducted on the clinical data of 60 patients with CHD admitted to the hospital from January 2021 to October 2023.All patients received carotid artery ultrasound examination and coronary angiography(CAG).With CAG as the gold standard,the patients were divided into coronary artery stenosis≥75%group(n=45)and coronary artery stenosis<75%group(n=15).The intima-media thickness of common carotid artery(IMTc)at 1 cm from the bifurcation,intima-media thickness at the bifurcation of carotid artery(IMTs)and plaque formation were compared between CHD patients with different degrees of coronary artery stenosis.The efficacy of carotid artery ultrasound parameters in evaluating the degree of coronary artery stenosis in patients with CHD was analyzed using the receiver operating characteristic(ROC)curve.Results IMTc,IMTs and plaque formation in CHD patients with coronary artery stenosis<75%were smaller/less than those in patients with coronary artery stenosis≥75%(P<0.05).When IMTc,IMTs,and plaque formation were used to evaluate the degree of coronary artery stenosis in patients with CHD,the AUC of plaque formation was the largest(0.889).Its sensitivity and specificity were 84.44%and 93.33%.Conclusion Carotid artery ultrasound parameters(IMTc,IMTs and plaque formation)are helpful for evaluating the degree of coronary artery stenosis in patients with CHD,and high specificity of plaque formation deserves clinical attention.
