眼部超声检查诊断早期视网膜裂孔的声像图特征及其价值分析

摘要 目的 探究眼部超声检查诊断早期视网膜裂孔的声像图特征及其价值。方法 纳入82例(82眼)于我院2020年12月至2022年11月就诊的疑似视网膜裂孔患者作为研究对象,部分患者行眼部超声检查,分析眼部超声检查结果及裂孔发生部位,手术结果为“金标准”,分析眼部超声检查诊断早期视网膜裂孔的应用价值,并计算一致性;分析早期视网膜裂孔的声像图特征。结果 “金标准”明确诊断视网膜裂孔患者71例(71眼),经眼部超声检查明确视网膜裂孔患者66例(66眼),包括巨大裂孔5例(5眼),发生率为7.2%(5/69);36例(36眼)干性马蹄形裂孔,发生率为52.2%(36/69);25例(25眼)为马蹄形裂孔合并视网膜脱离,发生率为36.2%(25/69)。眼部超声检查显示发生视网膜裂孔的66例患者裂孔分布于颞上方47例(71.2%),颞下方18例(27.3%),鼻侧1例(1.4%)。眼部超声检查在早期视网膜裂孔诊断中的灵敏度为90.1%(64/71),特异度为81.8%(9/11),准确度为89.0%(73/82),阳性预测值为97.0%(64/66),阴性预测值为56.2%(9/16)。kappa检验显示:眼部超声检查与“金标准”的一致性尚可(kappa值=0.60,P<0.001)。结论 眼部超声检查在早期视网膜裂孔诊断中应用价值较高,能够有效检出裂孔类型及部位,为临床进一步诊疗提供有效的参考,从而更好地遏制患者病情进展,值得推广应用。 Objective To explore the ultrasonographic features and value of eye ultrasound in diagnosing early retinal tears.Methods Eighty-two patients(82 eyes)with suspected retinal tears who visited our hospital from December 2020 to November 2022 were included as the study subjects.Some patients underwent eye ultrasound examination,and the results of eye ultrasound examination and the location of retinal tears were analyzed.The"gold standard"was the surgical results.The application value of eye ultrasound examination in diagnosing early retinal tears was analyzed,and consistency was calculated.Analyze the sonographic features of early retinal tears.Results The"gold standard"clearly diagnosed 71 patients(71 eyes)with retinal tears,and 66 patients(66 eyes)with retinal tears were confirmed through eye ultrasound examination,including 5 patients(5 eyes)with giant tears,with an incidence rate of 7.2%(5/69),36 cases(36 eyes)had dry horseshoe shaped hiatus,with an incidence rate of 52.2%(36/69),25 cases(25 eyes)had horseshoe shaped tears combined with retinal detachment,with an incidence rate of 36.2%(25/69).Eye ultrasound examination showed that retinal tears occurred in 66 patients,with 47 cases(71.2%)located above the temporal area,18 cases(27.3%)located below the temporal area,and 1 case(1.4%)located on the nasal side.The sensitivity of eye ultrasound examination in the early diagnosis of retinal tears is 90.1%(64/71),specificity is 81.8%(9/11),accuracy is 89.0%(73/82),positive predictive value is 97.0%(64/66),and negative predictive value is 56.2%(9/16).The kappa test showed that the consistency between eye ultrasound examination and the"gold standard"was still acceptable(kappa value=0.60,P<0.001).Conclusion Eye ultrasound examination has high application value in the early diagnosis of retinal tears,which can effectively detect the type and location of retinal tears,provide effective reference for further clinical diagnosis and treatment,and better control the progression of patients′conditions.It is worth promoting and applying.
