摘要
目的:探讨槐杞黄颗粒联合纤维支气管镜(FOB)治疗儿童肺炎支原体肺炎(MPP)的临床疗效及对患儿炎症因子、免疫功能等的影响。方法:选取2020年6月~2022年7月期间于某院治疗的90例MPP患儿作为研究对象,采用随机数字表法分为FOB组和联合组,每组45例。两组患儿入院后均给予祛痰、止咳、退热、补液等常规对症治疗,FOB组患儿给予FOB治疗,联合组在FOB组基础上口服槐杞黄颗粒,两组均连续治疗2周。比较两组患儿肺泡灌洗液中细胞因子[白介素-8(IL-8)、白介素-17(IL-17)、Toll样受体2(TLR2)]水平、免疫球蛋白[免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)]、T细胞亚群(CD3^(+)、CD8^(+)、CD4^(+)/CD8^(+))水平、临床疗效及不良反应发生情况。结果:治疗后,两组患儿IL-8、IL-17、TLR2水平均降低(P<0.05),且联合组低于FOB组(P<0.05);两组患儿IgA、IgG、IgM水平均升高(P<0.05),且联合组高于FOB组(P<0.05);两组患儿CD3^(+)、CD4^(+)/CD8^(+)水平均升高(P<0.05),且联合组高于FOB组(P<0.05);两组患儿CD8^(+)水平均降低(P<0.05),且联合组低于FOB组(P<0.05)。联合组患儿治疗总有效率(95.56%)高于FOB组(77.78%,P<0.05);两组患儿恶心、呕吐、刺激性咳嗽、胸闷、腹痛等不良反应总发生率比较无统计学差异(P>0.05)。结论:相比于单用FOB治疗,槐杞黄颗粒联合FOB治疗MPP患儿临床疗效更好,可有效缓解患儿临床症状,降低IL-8、IL-17、TLR2水平,提高免疫力。
Objective:To explore the clinical efficacy of Huaiqihuang granules combined with fiberoptic bronchoscopy(FOB)in the treatment of children with Mycoplasma pneumoniae pneumonia(MPP)and its effect on inflammatory factors and immune function.Methods:A total of 90 children with MPP who were treated in a certain hospital from June 2020 to July 2022 were divided into the FOB group and combination group by random number table method,with 45 patients in each group.Both groups were given conventional symptomatic treatment after admission such as expectorant,anti-tussive,antipyretic,fluid replacement therapies,the FOB group was given the FOB,the combination group was given Huaiqihuang granules in addition to the FOB,and both groups were treated continuously for 2 weeks.Thecytokines in alveolar lavage fluid[interleukin-8(IL-8),interleukin-17(IL-17),Toll-like receptor 2(TLR2)],immunoglobulins[immunoglobulin A(IgA),immunoglobulin G(IgG),immunoglobulin M(IgM)],T cell subsets(CD3^(+),CD8^(+),CD4^(+)/CD8^(+)),clinical response and adverse reactions were compared between the two groups.Results:After treatment,the levels of IL-8,IL-17 and TLR2 were decreased in both groups(P<0.05),and were lower in the combination group than those in the FOB group(P<0.05).The levels of IgA,IgG and IgM were increased in both groups(P<0.05),and were higher in the combination group than those in the FOB group(P<0.05).The levels of CD3^(+)and CD4^(+)/CD8^(+)were increased in both groups(P<0.05),and were higher in the combination group than those in the FOB group(P<0.05).The level of CD8^(+)was decreased in both groups(P<0.05),and was lower in the combination group than that in the FOB group(P<0.05).The total response rate in the combination group(95.56%)was higher than that in the FOB group(77.78%,P<0.05).There was no statistically significant difference in the total incidence of adverse reactions such as nausea,vomiting,irritant cough,chest tightness and abdominal pain between the two groups(P>0.05).Conclusion:Compared with FOB alone,Huaiqihuang granules combined with the FOB shows a definite clinical effect in the treatment of children with MPP with effectively relieved clinical symptoms,reduced levels of IL-8,IL-17 and TLR2,and improved immunity.
