Bcl-3、cyclin D1、P53、Ki-67蛋白在乳腺导管内增生性病变的表达及其意义

摘要 目的探讨B细胞淋巴瘤因子3(Bcl-3)、细胞周期蛋白D1(cyclin D1)、P53基因编码蛋白和增殖细胞核蛋白(Ki-67)在乳腺导管内增生性病变中的表达及应用价值。方法采用免疫组化检测乳腺普通型导管增生(UDH)、柱状上皮病变(CCL)、非典型导管增生(ADH)和导管原位癌(DCIS)组织中Bcl-3、cyclin D1、P53和Ki-67蛋白的表达。结果Bcl-3、cyclin D1、P53、Ki-67蛋白在UDH中均少量表达;在CCL、ADH和DCIS中均明显表达,且随着病变的级别升高,阳性率逐渐增高(P<0.05);Bcl-3、cyclin D1、P53和Ki-67蛋白在CCL、ADH和DCIS中着色强阳性(++~+++)的表达,差异有统计学意义(P<0.05)。结论Bcl-3、cyclin D1、P53和Ki-67蛋白的表达在乳腺导管内增生性病变的病理诊断、鉴别诊断及指导临床治疗方面有较高应用价值,可显著提高乳腺导管内增生性病变的诊断准确率。 Objective To investigate the expression and application value of Bcl-3,CyclinD1,P53 and Ki-67 in breast intraductal proliferative lesions.Methods Immunohistochemistry was used to detect the expression of Bcl-3,cyclin D1,P53 and Ki-67 proteins in the tissue of usual ductal hyperplasia(UDH),columnar cell lesion(CCL),atypical ductal hyperplasia(ADH)and ductal carcinoma in situ(DCIS).Results The expression of Bcl-3,cyclin D1,P53 and Ki-67 in UDH were slight.They were significantly expressed in CCL,ADH and DCIS,and the positive rate of the three groups increased gradually(P<0.05).The positive staining(++~+++)of Bcl-3,cyclin D1,P53 and Ki-67 in CCL,ADH and DCIS was statistically significant(P<0.05).Conclusion The expression of Bcl-3,cyclin D1,P53 and Ki-67 has high application value in the pathological diagnosis,differential diagnosis and clinical treatment of intraductal hyperplasia of breast,and can significantly improve the diagnostic accuracy of intraductal hyperplasia of breast.
