颅脑损伤患者术前铁蛋白表达及临床意义

摘要 目的分析颅脑损伤患者术前血清铁蛋白(SF)的表达水平,探讨其与患者病情及预后的关系。方法采用前瞻性研究,选择2022年1—12月于郑州市第七人民医院行血肿清除及去骨瓣减压术治疗的116例颅脑损伤患者作为研究对象,纳入颅脑损伤组。根据格拉斯哥昏迷评分(GCS)将患者分为轻型昏迷组、中型昏迷组及重型昏迷组;根据术后1个月格拉斯哥预后评分(GOS)将患者分为预后良好组与预后不良组;另外选择医院健康体检的50例健康志愿者,纳入对照组。检测患者术前SF水平,采用Pearson相关分析颅脑损伤患者术前SF表达与GCS、GOS评分的相关性;同时绘制受试者工作特征(ROC)曲线,探究术前SF对颅脑损伤患者预后的预测价值。结果116例颅脑损伤患者术后预后不良37例,占比31.90%;预后良好79例,占比68.10%。颅脑损伤组血清SF水平高于对照组(P<0.05)。不同昏迷程度患者SF均高于对照组(P<0.05);重型昏迷组SF高于轻型昏迷组和中型昏迷组,中型昏迷组SF高于轻型昏迷组(P<0.05)。预后不良组血清SF水平高于预后良好组与对照组(P<0.05);预后良好组SF水平高于对照组(P<0.05)。经Pearson相关分析,术前SF表达与GCS、GOS评分呈负相关(r<0,P<0.05)。绘制ROC曲线,结果显示,术前SF表达预测颅脑损伤患者预后不良的曲线下面积(AUC)>0.7,有一定的预测价值。结论颅脑损伤患者术前SF呈现高表达,与颅脑损伤严重程度及预后有相关性,且术前SF可以作为颅脑损伤患者预后评估的参考指标。 Objective To analyze the expression level of serum ferritin(SF)in patients with traumatic brain injury before surgery,and explore its relationship with the patient’s condition and prognosis.Methods A prospective study was conducted,116 patients with traumatic brain injury who underwent hematoma clearance and decompressive craniectomy at the 7th People’s Hospital of Zhengzhou City from January 2022 to December 2022 were selected as the study subjects and were included in the traumatic brain injury group.According to the Glasgow coma scale(GCS),patients were divided into mild coma group,moderate coma group,and severe coma group.According to the Glasgow outcome scale(GOS)at one month after surgery,patients were divided into a good prognosis group and a poor prognosis group.In addition,50 healthy volunteers who underwent hospital health examinations were selected and included in the control group.Pearson correlation analysis was used to analyze the correlation between preoperative SF expression and GCS score and GOS score in patients with traumatic brain injury.At the same time,the receiver operating characteristic(ROC)curve was drawn to explore the predictive value of preoperative SF on the prognosis of patients with traumatic brain injury.Results Among the 116 patients with traumatic brain injury,37 had poor postoperative prognosis,accounting for 31.90%.There were 79 cases had a good prognosis,accounting for 68.10%.The SF level in the traumatic brain injury group was higher than that in the control group(P<0.05).The SF levels of patients with different levels of coma were higher than those of the control group(P<0.05).The SF of the severe coma group was higher than that of the mild coma group and the moderate coma group,and the serum SF of the moderate coma group was higher than that of the mild coma group(P<0.05).The SF level in the poor prognosis group was higher than that in the good prognosis group and the control group(P<0.05).The SF level in the good prognosis group was higher than that in the control group(P<0.05).According to Pearson correlation analysis,preoperative SF expression was negatively correlated with GCS and GOS score(r<0,P<0.05).The ROC curve was plotted,and the results showed that the area under the curve(AUC)of preoperative SF expression predicting poor prognosis in patients with traumatic brain injury was greater than 0.7,which had certain predictive value.Conclusion Preoperative SF in patients with traumatic brain injury shows high expression,which is correlated with the severity and prognosis of the injury.Preoperative SF can be used as a reference indicator for evaluating the prognosis of patients with traumatic brain injury.
