无创呼吸机对急性心力衰竭合并呼吸衰竭患者心肺功能及肌酸激酶同工酶(CK-MB)的影响分析

摘要 目的评估无创呼吸机应用在急性心力衰竭(AHF)合并呼吸衰竭(RF)患者治疗中的效果及对心肺功能、肌酸激酶同工酶(CK-MB)的影响。方法研究时间段2021年1月至2022年8月,研究对象80例AHF合并RF患者,分组方法:随机数字表法[对照组(40例,行常规药物治疗)、观察组(40例,加用无创呼吸机治疗)],评价组间治疗有效性、血气结果、心肺功能、血清学指标。结果(1)较之对照组(75.00%),观察组治疗有效率(95.00%)更高,P<0.05;(2)治疗前,组间血气结果[酸碱度(pH)、动脉血氧饱和度(SaO_(2))、动脉血氧分压(PaO_(2))]、心肺功能[心率(HR)、心脏指数(CI)、左室射血分数(LVEF)、用力肺活量(FVC)、最大呼气容量(PEF)、1(LVEF)、用力肺活量(FVC)]、血清学指标[CK-MB、氨基末端B型钠尿肽原(NT-pro BNP)、心肌肌钙蛋白I(cTnI)]无差异,P>0.05;治疗后,观察组血气结果(pH、SaO_(2)、PaO_(2))、心肺功能(HR、CI、LVEF、FVC、PEF、FEV1)、血清学指标(CK-MB、NT-pro BNP、cTnI)更佳,P<0.05。结论对AHF合并RF患者行无创呼吸机治疗,能够改善心肺功能、血气分析结果,降低CK-MB、NT-pro BNP、cTnI水平,提高治疗有效性,值得推广。 Objective To evaluate the effectiveness of non-invasive ventilation in the treatment of patients with acute heart failure(AHF)and respiratory failure(RF),as well as its impact on cardiopulmonary function and creatine kinase isoenzyme(CK-MB).Method A total of 80 patients with AHF complicated with RF from January 2021 to August 2022 were included.They were randomly divided into a control group(40 cases included,treated with conventional medication)and an observation group(40 cases included,treated with non-invasive ventilation)using a random number table method.The treatment effectiveness,blood gas results,cardiopulmonary function,and serological indicators between the groups were evaluated.Result(1)Compared with the control group(75.00%),the observation group had a higher treatment effective rate(95.00%),with P<0.05;(2)Before treatment,there was no difference in inter group blood gas results[pH,arterial oxygen saturation(SaO_(2)),arterial partial pressure of oxygen(PaO_(2))],cardiopulmonary function[heart rate(HR),cardiac index(CI),left ventricular ejection fraction(LVEF),forced vital capacity(FVC),maximum expiratory volume(PEF),forced expiratory volume at 1 second(FEV1)],serum indicators[CK-MB,N-terminal B-type natriuretic peptide(NT pro BNP),and cardiac troponin I(cTnI)],with P>0.05;After treatment,the observation group showed better blood gas results(pH,SaO_(2),PaO_(2)),cardiopulmonary function(HR,CI,LVEF,FVC,PEF,FEV1),and serum indicators(CK-MB,NT pro BNP,cTnI),with P<0.05.Conclusion Non invasive ventilation therapy for patients with AHF combined with RF can improve cardiopulmonary function,blood gas analysis results,reduce CK-MB,NT pro BNP,cTnI levels,and enhance treatment effectiveness,which is worth promoting.
