摘要
目的:探究艾司氯胺酮联合瑞芬太尼、丙泊酚麻醉对输尿管支架取出术患者疼痛程度及术后苏醒的影响。方法:选择2022年6月—2023年6月于海安市中医院行输尿管支架取出术患者95例。采用随机数表法分为对照组(n=47)和研究组(n=48),对照组给予瑞芬太尼+丙泊酚麻醉,研究组给予瑞芬太尼+丙泊酚+艾司氯胺酮麻醉。分析并比较两组麻醉效果、疼痛程度、苏醒质量、认知功能和不良反应发生率。结果:研究组麻醉总优良率为95.83%,明显高于对照组的80.85%,差异有统计学意义(P<0.05)。研究组视觉模拟评分法(VAS)评分明显低于对照组,苏醒时间早于对照组,差异有统计学意义(P<0.05)。术后1 h、3 h,研究组简易智力状态检查量表(MMSE)评分显著高于对照组,差异有统计学意义(P<0.05)。研究组不良反应总发生率为8.33%,低于对照组的14.89%,但两组间比较,差异无统计学意义(P>0.05)。结论:艾司氯胺酮联合瑞芬太尼、丙泊酚麻醉可显著减轻输尿管支架取出术患者的疼痛程度,降低认知功能损害,缩短术后苏醒时间,有较高的安全性。
Objective:To investigate the effect of Esketamine combined with Remifentanil and Propofol anaesthesia on pain levels and postoperative awakening in patients undergoing ureteral stent removal.Method:A total of 95 patients who underwent ureteral stent removal in Hai’an Hospital of Traditional Chinese Medicine from June 2022 to June 2023 were selected.Random number table method was used to divide the control group(n=47)and the study group(n=48).The control group was given Remifentanil+Propofol anesthesia,and the study group was given Remifentanil+Propofol+Esketamine anesthesia.Anesthesia effect,pain degree,quality of recovery,cognitive function and incidence of adverse reactions were analyzed and compared between the two groups.Result:The total excellent and good rate of anesthesia in the study group was 95.83%,which was significantly higher than 80.85%in the control group,and the difference was statistically significant(P<0.05).The visual analogue scale(VAS)score of the study group was significantly lower than that of the control group,and the recovery time was earlier than that of the control group,the differences were statistically significant(P<0.05).At 1 h and 3 h after operation,the score of mini-mental state examination(MMSE)in the study group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).The total incidence of adverse reactions in the study group was 8.33%,which lower than 14.89%in the control group,but there was no significant difference between the two groups(P>0.05).Conclusion:Esketamine combined with Remifentanil and Propofol can significantly reduce the pain of patients undergoing ureteral stent removal,reduce cognitive impairment,shorten postoperative recovery time,and have a high safety.
