摘要
目的:观察研究温肾健脾利水方联合中药穴位贴敷治疗脾肾气虚型慢性肾小球肾炎的临床效果。方法:选择2022年4月—2023年4月成都中医药大学附属医院收治的86例脾肾气虚型慢性肾小球肾炎患者作为研究对象,按随机数表法将其分为对照组(常规西医治疗)与研究组(在对照组基础上应用温肾健脾利水方联合中药穴位贴敷),各43例。比较两组临床疗效、肾功能指标、血清炎症因子水平、体液免疫指标及细胞免疫指标。结果:研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗后,两组24 h尿蛋白定量、血清肌酐(Scr)、血尿素氮(BUN)、血清肿瘤坏死因子-α(TNF-α)及白细胞介素-6(IL-6)水平较治疗前降低,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后,两组血清免疫球蛋白A(IgA)、CD8^(+)水平较治疗前降低,但血清免疫球蛋白G(IgG)、血清免疫球蛋白M(IgM)、CD4^(+)、CD4^(+)/CD8^(+)较治疗前升高,且研究组优于对照组,差异有统计学意义(P<0.05)。结论:温肾健脾利水方联合中药穴位贴敷能提高脾肾气虚型慢性肾小球肾炎患者的治疗效果,改善肾功能,降低血清炎症因子水平,纠正免疫机制失衡状态。
Objective:To observe and study the clinical effect of Wenshen Jianpi Lishui Formula combined with traditional Chinese medicine acupoint application in the treatment of chronic glomerulonephritis of spleen and kidney Qi deficiency type.Method:A total of 86 patients with chronic glomerulonephritis of spleen and kidney Qi deficiency type who treated in Affiliated Hospital of Chengdu University of Traditional Chinese Medicine from April 2022 to April 2023 were selected as the study objects,and they were randomly divided into the control group(treated with conventional western medicine)and the study group(treated with Wenshen Jianpi Lishui Formula combined with traditional Chinese medicine acupoint application on the basis of the control group),with 43 cases in each group.The clinical efficacy,renal function indicators,serum inflammatory factor levels,humoral immune indicators and cellular immune indicators of two groups were compared.Result:The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05);after treatment,the levels of 24 h urinary protein quantification,serum creatinine(Scr),blood urea nitrogen(BUN),serum tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)in two groups were lower than those before treatment,and those in the study group were lower than those in the control group,the differences were statistically significant(P<0.05);after treatment,the levels of serum immunoglobulin A(IgA)and CD8^(+)in two groups were lower than those before treatment,but the levels of serum immunoglobulin G(IgG),serum immunoglobulin M(IgM),CD4^(+)and CD4^(+)/CD8^(+)were higher than those before treatment,and those in the study group were better than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Wenshen Jianpi Lishui Formula combined with traditional Chinese medicine acupoint application can improve the treatment effect of chronic glomerulonephritis of spleen and kidney Qi deficiency type,improve renal function,reduce serum inflammatory factor levels,correct immune mechanism imbalance.
