摘要
目的观察前列腺电切术患者采取知信行模式的3H护理联合自我清洁间歇导尿术干预后的效果。方法纳入本院2022年5月—2023年6月开展前列腺电切术患者共96例,随机分为两组,每组48例。对照组予以常规护理措施及留置导尿,观察组予以知信行模式的3H护理和自我清洁间歇导尿术联合干预方案。基于膀胱功能、留置尿管舒适度、情绪状态、生活质量进行综合评估。结果出院时,两组膀胱压力、最大膀胱容量较术后1 d均提高,膀胱残余尿量均减少(P<0.05),观察组膀胱压力、膀胱最大容量较对照组更高,膀胱残余尿量较对照组更少(均P<0.05)。干预后,两组汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)较干预前均减少(P<0.05),且观察组HAMA、HAMD得分较对照组更低(均P<0.05)。比较两组留置尿管舒适度分级,观察组0度(无不适)人数比例高于对照组(P<0.05)。干预后,两组生存质量测定量表(WHOQOL-BREF)较干预前均增加(P<0.05),且观察组WHOQOL-BREF得分较对照组更高(P<0.05)。结论前列腺电切术患者采取知信行模式的3H护理联合自我清洁间歇导尿术干预后可明显改善膀胱功能,提高留置尿管期间舒适度,缓解负性情绪,提升生活质量。
Objective To observe the effect of 3H nursing combined with self-cleaning intermittent catheterization in patients undergoing electroprostatectomy.Methods A total of 96 patients undergoing electroprostatectomy in our hospital from May 2022 to June 2023 were randomly divided into two groups with 48 cases in each group.The control group was given routine nursing measures and indwelling catheterization,and the observation group was given 3H nursing and self-cleaning intermittent catheterization combined intervention program.Comprehensive assessment was made based on bladder function,indwelling catheter comfort,emotional state and quality of life.Results At the time of discharge,the bladder pressure and maximum bladder volume in both groups were increased,and the residual urine volume in both groups was decreased(P<0.05).The bladder pressure and maximum bladder volume in the observation group were higher than those in the control group,and the residual urine volume in the observation group was lower than those in the control group(all P<0.05).After intervention,the Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)in both groups were reduced compared with those before intervention(P<0.05),and the HAMA and HAMD scores in the observation group were lower than those in the control group(both P<0.05).Comparing the comfort grade of indwelling catheter between the two groups,the proportion of 0 degree(no discomfort)in observation group was higher than that in control group(P<0.05).After intervention,the quality of life scale(WHOQOL-BREF)of both groups was increased compared with that before intervention(P<0.05),and the WHOQOL-BREF score of the observation group was higher than that of the control group(P<0.05).Conclusions 3H nursing combined with self-cleaning intermittent catheterization can significantly improve bladder function,improve comfort during indwelling catheter,relieve negative emotions and improve quality of life in patients undergoing electroprostatectomy.
