摘要
目的:探究百令胶囊联合甲巯咪唑治疗老年甲状腺功能亢进症(甲亢)的效果。方法:选择2020年12月—2022年12月在泰安市妇幼保健院治疗的老年甲亢患者94例,应用随机数字表法将其分为对照组(甲巯咪唑)及观察组(联合百令胶囊)各47例。两组均治疗28 d。对比两组治疗总有效率、甲状腺功能[游离甲状腺三碘原氨酸(FT_(3))、游离甲状腺素(FT_(4))、促甲状腺素(TSH)、甲状旁腺激素(PTH)]、血清细胞因子[白细胞介素-4(IL-4)、白细胞介素-17(IL-17)、转化生长因子-β(TGF-β)、γ干扰素(IFN-γ)]、氧化应激水平[超氧化物歧化酶(SOD)、丙二醛(MDA)、谷胱甘肽过氧化物酶(GPX)]、不良反应发生率。结果:对照组治疗总有效率低于观察组,差异有统计学意义(P<0.05)。治疗前,两组FT_(3)、FT_(4)、TSH、PTH水平对比差异均无统计学意义(P>0.05);治疗后,两组FT_(3)、FT_(4)、PTH水平均降低,TSH均升高,对照组FT_(3)、FT_(4)、PTH水平均高于观察组,TSH低于观察组,差异均有统计学意义(P<0.05)。治疗前,两组IL-4、IL-17、TGF-β、IFN-γ水平对比差异均无统计学意义(P>0.05);治疗后,两组IL-4、IL-17、IFN-γ水平均降低,TGF-β水平均升高,观察组IL-4、IL-17、IFN-γ水平均低于对照组,TGF-β水平高于对照组,差异均有统计学意义(P<0.05)。治疗前,两组SOD、MDA、GPX水平对比差异均无统计学意义(P>0.05);治疗后,两组MDA均降低,SOD、GPX均升高,对照组MDA高于观察组,SOD、GPX均低于观察组,差异均有统计学意义(P<0.05)。对照组不良反应发生率与观察组对比差异无统计学意义(P>0.05)。结论:老年甲亢患者采用百令胶囊联合甲巯咪唑治疗,可改善甲状腺功能,降低炎症反应,提高抗氧化应激能力,安全性较高。
Objective:To investigate the effect of Bailing Capsule combined with Methimazole in the treatment of elderly hyperthyroidism.Method:A total of 94 cases with elderly hyperthyroidism treated in Tai’an Maternal and Child Health Hospital from December 2020 to December 2022 were selected and divided into 47 cases each in the control group(Methimazole)and the observation group(combined with Bailing Capsule)by the randomized table method.Both groups were treated for 28 days.The total effective rate,thyroid function[free thyroid triiodothyronine(FT_(3)),free thyroxine(FT_(4)),thyroid stimulating hormone(TSH),parathyroid hormone(PTH)],serum cytokines[interleukin-4(IL-4),interleukin-17(IL-17),transforming growth factor-β(TGF-β),interferon-γ(IFN-γ),oxidative stress level[superoxide dismutase(SOD),malondialdehyde(MDA),glutathione peroxidase(GPX)],and incidence of adverse reactions in two groups were compared.Result:The total effective rate of the control group was lower than that of the observation group,the difference was statistically significant(P<0.05).Before treatment,there were no statistically significant differences in FT_(3),FT_(4),TSH and PTH levels between the two groups(P>0.05).After treatment,the levels of FT_(3),FT_(4) and PTH in the two groups were decreased,and levels of TSH in the two groups were increased;the levels of FT_(3),FT_(4) and PTH in the control group were higher than those in the observation group,and TSH was lower than that in the observation group,the differences were statistically significant(P<0.05).Before treatment,there were no statistically significant differences in the levels of IL-4,IL-17,TGF-βand IFN-γbetween the two groups(P>0.05).After treatment,the levels of IL-4,IL-17 and IFN-γin the two groups were decreased,and the levels of TGF-βin the two groups were increased;the levels of IL-4,IL-17 and IFN-γin the observation group were lower than those in the control group,and the level of TGF-βwas higher than that in the control group,the differences were statistically significant(P<0.05).Before treatment,there were no statistically significant differences in SOD,MDA and GPX levels between the two groups(P>0.05).After treatment,MDA in the two groups decreased,SOD and GPX increased,MDA in the control group was higher than that in the observation group,SOD and GPX were lower than those in the observation group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the control group and the observation group(P>0.05).Conclusion:Elderly patients with hyperthyroidism can be treated with Bailing Capsule combined with Methimazole,which can improve thyroid function,reduce inflammatory response,improve the anti-oxidative stress ability,and have higher safety.
